- The response date is extended to July 20, 2007 at 3:00 pm.ation to omit the reference to obtaining vital statistics here? Vital statistics are normally obtained either before or after the death call, and rarely during a distraught late-night call. Ideally, USU should obtain most vitals when the donation is pre-arranged. When the contractor is engaged to obtain vitals, it should be able to assess an additional fee for that work. So can you list "Obtaining Vital Statistics" as a separate fee? ANSWER: CLIN 0001AV is amended to read as: After hour Death Calls 12 ea Clin 0001AW is added to read as: Obtaining Vital Statistics 6 ea 8. QUESTION: Can the solicitation be amended to list fees for the following: --"Securing a signed death certificate," both within and outside a 50-mile radius, ANSWER: Securing death certificates in covered in Permits of the SOW. --"Other extra trip/stop, as required," both within and outside a 50-mile radius? ANSWER: A Clin will be added for any additional trip/stops. 9. Clin 0001AS is deleted. 10.
