Description: This is combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-07-T-0233. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect t hrough Federal Acquisition Circular 2005-07. The North American Industry Classification System Code (NAICS) for this procurement is 811219. The Fox Army Health Center, Redstone Arsenal, Alabama 35809, has a requirement for the following: Clin 0001: Annual Service Contract for AGFA CR-75 Computed Radiology System SN#2122 and 2ea AGFA 5300 Digital Laser Printers and all related components that make up the Computed Radiology System beginning October 2007 thru Sep 2008. Clin 0002: Contractor Manpower Reporting - The contractor is required to report the following contractor manpower information, associated with performance of this contract action in support of Army requirements, for all covered contracts, to the Office, A ssistant Secretary of the Army (Manpower and Reserve Affairs) (ASA(M&RA)), using the secure Army data collection web site at https://cmra.amy.mil/default.aspx (Other information requirements associated with the manpower data collection (contract and task o r delivery order numbers; appropriation data and amounts; total estimated value of contract; federal supply class or service code; major Army organizational element receiving or reviewing work; beginning and ending date for reporting period; place of perfo rmance; name, address, and point of contract for contractor; etc.) are specified and explained at the web site.)
