- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81NTE71908513 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18. This acquisition is solicited as a 100% Small Business set-aside for NAICS 339112, size 500 employees. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand name or Equal applies to this acquisition. The requirements of this soli citation are: Item no. 0001: MPS 2 Myocardial Protection System which includes the following hardware and accessories: 1 EA #5001260 MPS 2 Console; 1 EA #5001001 MPS Hypothermic Reservoir; 1 EA #5001002, MPS Hypothermic Reservoir Holder; 1 EA #5001003 M PS Ice Reservoir Hoses; 1 EA #5001110 MPS Water Circuit Adapter; 1 EA #5201010, MPS Operation Manual, 1 EA #511600A, Aortic Root Transducer Interface Cable; 1 EA #511600R, Coronary Sinus Transducer Interface Cable, 1 EA #520NA16, MPS Power Cord, (length 16 FT). MFG: Quest Medical Inc., BRAND NAME OR EQUAL SPECIFICATIONS. Interested firms shall comply with FAR clause 52.211-6, Brand Name or Equal, if quoting other than the brand name product. Clause 52.211-6 and other clauses cited throughout this solici tation can be viewed by accessing website: http://farsite.hill.af.mil. Quotes shall include all shipping and handling charges (FOB Destination) for delivery to Brooke Army Medical Center, Building 3600, 3851 Roger Brooke Drive, Fort Sam Houston, TX 78234 -6200. Vendor shall indicate on their price quote if shipping method will be other than FOB Destination. NOTE: THIS IS AN UNFUNDED REQUIREMENT AND IS SUBJECT TO AVAILABILITY OF FUNDS. FUNDS AVAILABILITY: Funds are not currently available for this requirement, however, it is anticipated year-end funds may become available. Therefore, all quotes submitted in response to this RFQ must remain firm through 30 Sep 07. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) pro gram. The CCR can be accessed via the internet at www.ccr.gov/ . Confirmation of CCR registration will be validated prior to awarding a contract. The Government will award a contract resulting from this combined synopsis/solicitation to the responsible firm whose quote is lowest priced technically acceptable. Technical acceptability is compliance with the requirements of FAR Clause 52.211-6 , if qu oting on an equal product.
