- Special Instructions Provide all necessary labor, equipment, and materials and perform all operations necessary for paving the Patoka Lake office entrance road as in accordance with the specifications dated 09 August 2007. All work is to be completed at the Patoka Lake Office ; 4512 N. Cuzco Road South; Dubois, IN 47527 This procurement is 100% set-aside for Small Business, NAICS 238990 ($13.0 Million). There is no bonding requirement for this action. Delivery shall be FOB Destination. The contract award will be based on lowest quote and CCR registration. All information technology purchase must meet the standards in accordance with Section 508 of the Rehabilitation Act of 1973.
