- Description
- This is a combined synopsis/solicitation for nonpersonal health care services as defined in FAR 37-101, under which the contractor is an independent contractor. This announcement constitutes the only solicitation; proposals are being requested; a written solicitation is attached for ease in submitting a bid only. The solicitation number as shown in this announcement is a Request for Quotation (RFQ) W912LQ-07-T-0036. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-18. This solicitation is set aside for small business with NAICS of 621999 with a size standard of $8.5 million. The contractor will provide mobile medical surveillance/occupational health services for approximately 275 Virginia Army National Guard personnel who are exposed to health hazards in the work environment or who are assigned to positions requiring specific standards of physical fitness. Some personnel will receive comprehensive exams; others will receive limited testing at two on-site facilities in Virginia. The contractor agrees to provide the following services (quantities are approximate): CLIN 0001AA: 145 each Physical Examinations $ ________; CLIN 0001AB: 3 each Physical Examinations with DOT certification/documentation $ ________; CLIN 0001AC: 145 each Audiometric Testing $ ________; CLIN 0001AD: 25 each Pulmonary Function Testing $ ________; CLIN 0001AE: 65 each Electrocardiograms (12 lead) $ ________; CLIN 0001AF: 10 each Blood Lead and ZPP Levels $ ________; CLIN 0001AG: 25 each Respirator Fit Test (Quantitative) $________; CLIN 0001AH 145 each Lab analysis $________. The Government may evaluate the quality of the professional and administrative services provided by the contractor, but retains no control over the medical, professional aspects of services rendered. The contractor is required to indemnify the Government for any liability producing act or omission by the contractor, its employees and agents occurring during contract performance. The contractor is required to maintain medical liability insurance in a coverage amount not less than the amount normally prevailing within the local community. In addition, any subcontracts for provisions of health care services must contain the requirements for clause 52.237-7. This contract will be a firm fixed price contract for one base year with one option year. Option year pricing will be adjusted using the difference between the 2007 and 2006 Annual Producer Price Index for General Medical and Surgical Hospitals. Award will be made to the Contractor whose proposal, responsive and conforming to the Request for Quote (RFQ), is determined to be the best value to the government. Reference is made to clause 52.212-2 Evaluation - Commercial Items; the following factors shall be used to evaluate offers: 1) technical: staff, work plan to supervise practitioners, mobile unit, equipment; 2) Past performance: vendor is to provide names and contact information for three facilities/organizations where similar services have been provided; 3) Price.
