- DESCRIPTION: VA Illiana Health Care System, 1900 East Main Street, Danville, IL 61832 is seeking vendors to provide mobile Extracorporeal Shock Wave Lithotripsy equipment and related technical support to veterans that qualify as beneficiaries of the VAIHCS upon forty-eight hours advance notice by VAIHCS. VAIHCS anticipates that an average of one veteran will be treated per scheduled visit but may be as low as one or as high as six. VAIHCS will furnish any/all hookups required. Evaluation of proposals will be based on Management and Technical Capability, Past Performance and Price. Management and Technical Capability, and Past Performance, when combined, are more important than price. This notice is open to qualified sources of small businesses relative to 2002 NAICS Code 621999 (size standard $9.0 million). The solicitation is 100% Total Small Business Set-Aside. Offerors MUST be registered in the Central Contractor Registration at http://www.ccr.gov. Offerors MUST ensure registration in this website is correct and current. The intended contract will be for a base year with the Government's right to extend the contract in one-year increments for a possibility of up to four option years. All offerors are advised to thoroughly review the entire document, paying special attention to all documentation required to be submitted in the proposal. The solicitation documents will be posted at www.fedbizopps.gov on or about August 6, 2007 with a proposal due date of August 31, 2007, 3:30PM local time. Amendments to the solicitation will be posted at www.fedbizopps.gov. Hard copies of the amendments will NOT be individually mailed. By registering to "Receive Notification" at www.fedbizopps.gov , you will be notified by e-mail of any new amendments that have been issued and posted. No other notification of amendments will be provided. Offerors are advised that they are responsible for obtaining all amendments to this solicitation. If any vendor prefers to have a hard copy of the solicitation (amendments must be downloaded from www.fedbizopps.gov) sent to them as opposed to downloading and printing a copy from this site, a written request must be sent to the Contracting Officer, faxed requests are acceptable; telephone requests will NOT be accepted.
