- Description: The Aleda E. Lutz VAMC, Saginaw, Michigan has a requirement for a contractor or contractors to furnish all management, supervision, labor, equipment, supplies and transportation necessary to perform BACK-UP Laboratory Services which may include STAT and Routine clinical diagnostic laboratory testing 24/7, 365 days per year, including all Federal holidays. Contractor shall provide any laboratory test properly authorized and requested by a VA-authorized physician. Contractor shall provide services within a geographical area which allows for the performance of testing and reporting within specified time frames (90 minutes for STAT tests, 4 hours for Non-STAT tests dropped off at contractor's site, 2 hours for STAT, 4 1/2 hours for Non-STAT if picked up by courier). Contractor may offer terms for courier service as a separate line item. Contractor(s) shall be approved to provide clinical diagnostic laboratory tests under both Medicare and Medicaid regulations and all applicable federal and state laws. To be eligible for award, the contract clinical laboratory shall also be fully accredited by the College of American Pathologists (CAP), Department of Health & Human Services HCFA Clinical Laboratory Improvement Amendment (CLIA), and/or the Joint Commission on the Accreditation of Hospital Organizations (JCAHO). All personnel assigned to perform services under this contract shall be licensed in a state, territory, or commonwealth of the United States or District of Columbia and shall maintain a full and unrestricted license at all times. The contract period shall be from the date of award (approximately July 1, 2007) through June 30, 2008 with (4) one year option renewals through May 31, 2011. This procurement is a full and open competition. However, the NAICS code is 621511 and the Small Business Standard is $11.5 average annual receipts for the preceding three years. This solicitation will be conducted in accordance with FAR Part 12 and FAR Part 15. Solicitation documents should be available on or about May 08, 2007. Anticipated offer due date is June 5, 2007 at 3:00 P.M. (DST).
