Description
This is a combined synopsis/solicitation for non-personal services prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for proposal (RFP) for solicitation FA4890-07-T-0012. No formal solicitation will be issued; however, a complete performance work statement (PWS) and Schedule B (Bid Schedule) will be posted shortly after this combined synopsis/solicitation. The solicitation document and FAR incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-17; Air Force Federal Acquisition Regulation Supplement, Air Force Acquisition Circular (AFAC) 2007-0531; and Defense Federal Acquisition Regulation, Defense Change Notice (DCN) 20070531. The proposed acquisition is available for full and open competition. The NAICS code is 713940 and the small business size standard is $6.5M. This is a Firm Fixed Price acquisition with a basic period of performance from 17 Aug 07 ? 16 Aug 08 and four option years starting 17 Aug 08. The objective of this contract is to provide membership to a commercial fitness facility similar to the Shell Bank Fitness Center on Langley AFB for approximately 125 Acquisition Management and Integration Center (AMIC) personnel in accordance with the Performance Work Statement ?AMIC Fitness Services? dated 7 July 2007. PERFORMANCE WORK STATEMENT In order to ensure the strength and stamina of AMIC personnel to complete their respective missions successfully and maintain strict USAF physical standards, the Department of Defense provides necessary physical fitness facilities and training areas. Maintaining the physical fitness of AMIC personnel is essential to keeping a healthy force ready for combat efficiency and work performance in defense of the United States of America. Military fitness standards are outlined in Air Force Instruction (AFI) 10-248. In accordance with AFI 10-248 fitness facilities contribute to Air Force (AF) readiness by enhancing the physical and mental stamina of military and civilian personnel. This is accomplished by providing a variety of programs that contribute to fitness. II. SCOPE: The contractor shall provide membership benefits to personnel assigned to the USAF AMIC, VA. Eligible participants are defined as all active duty and civilian personnel whose duty station is located in the SunTrust Bank Building 11817 Canon Blvd, Newport News, VA. 23606-4516. III. MINIMUM REQUIREMENTS OF THE PHYSICAL FITNESS FACILITY The Fitness Center must have accommodations similar to the Langley AFB Shellbank Fitness Center. In order to support the USAF AMIC mission effectively, the contractor?s fitness staff must be knowledgeable of preventative health maintenance and weight management. The contractor?s facility must offer core fitness programs that contribute to cardiovascular fitness, strength conditioning, and flexibility, i.e. total fitness. As a minimum, the following fitness programs/equipment must be offered: cardiovascular equipment, strength conditioning equipment, aerobic, cycling, walking, and jogging programs. The facility shall have clean, well ventilated, operating showers and separate men/women locker rooms. The commercial fitness facility shall be capable of providing an indoor room/space large enough to allow 55 members to safely participate in group activities such as calisthenics, aerobics, volleyball or basketball (time to be determined by the government). The facility shall be located within 3 miles of 11817 Canon Blvd, Newport News, VA. 23606-4516 to allow members sufficient time to workout and return to work in the allotted amount of time in accordance with Air Force guidelines. IV. FULL MEMBERSHIP BENEFITS A. Membership in the fitness facility shall include: 1. All standard commercial services offered by the fitness facility, 2. Unlimited access during normal operating hours, 3. Additional discounts normally extended to members and 4. Provide personnel training plan and advice to members initially and as required. B. Membership does not include: 1. The use of health spas, massages, racquet clubs, and golf courses, bowling centers, sporting clubs, and swimming facilities unless included in a standard commercial membership. V. MEMBERSHIP CARDS. Membership cards issued by the contractor shall identify the name of the government participant along with his or her unit. Any replacement fee for lost cards is the responsibility of the participant. VI. PROCEDURES A. Memberships shall run from the first day of the month for a 12-month period. The members will register through their respective USAF AMIC Fitness Program Manager and the contractor?s fitness staff. Each customer will review the membership rules and submit application through the contractor?s fitness staff. The contractor will issue commercial membership cards to each member. B. The contractor shall submit a monthly members usage report NLT five days from the end of each month to the AMIC Fitness Program Manager. C. The fitness facility shall submit invoice to the AMIC Fitness Program Manager. Monthly invoices must identify the member?s names or card numbers being billed. The Government shall not be responsible for making a payment until the AMIC Fitness Program Manager receives a completed list by name or card number with the invoice of services. D. The contractor shall submit a monthly invoice to the AMIC Fitness Program Manager including regular membership member usage report. E. IMPORTANT! The contractor will only submit an invoice for members who have attended the fitness facility at least one time during the month being invoiced. The AMIC Fitness Program Manager will manage membership cards and a list of active participants will be provided to the contractor. The AMIC Fitness Program Manager will update the active participant listing quarterly. The Government shall not be responsible for making a payment until the AMIC Fitness Program Manager receives a completed list by name with the invoice of services. Additional memberships may be added to the list as required. F. Military and civilian participants may be temporarily suspended due to military commitments. The contractor will be provided documentation to support the military member?s absence. (CCR) (http://www.ccr.gov/) in order to receive the award. The following provisions and clauses apply to this solicitation: 52.212-1, Instructions to Offers-Commercial Items; As a minimum , offerors must show (1) Solicitation number; (2) The name, address, and telephone number of the offeror; (3) A technical description of services being offered in sufficient detail to evaluate compliance with the requirements in the solicitation; (4) Price and any discount terms; (5) Remit to address if different than mailing address; (6) Past performance information (maximum of three), to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); 52.212-2 Evaluation ? Commercial Items ? The government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors shall be used to evaluate offeror?s: price, technical capability offered to meet the Government?s requirement, and past performance. Award will be based on the lowest price technically acceptable, best value to the government from a responsible quote as defined in FAR Part 9, who also has satisfactory record of past performance. Technical and past performance, when combined, are equal to price. Evaluation Factors are as follows: 1) Price: Submit a firm-fixed-price quote for the effort outlined in the Performance Work Statement. Price should clearly state the amount each member will be charged for a full month membership. 2) Technical Approach. Request you prepare a Technical Approach that includes information that demonstrates your technical capability and proven past performance to successfully complete the requirement as outlined in the PWS (Maximum of five (5) pages); Illustrate or discuss the project execution, example: the issue of membership cards, an updated computer system capable of maintaining separate membership and printing a report monthly. (Maximum of five (5) pages). 3) Past Performance: submit three (3) references of similar work performed in the past three years. References should include contract numbers, performance periods, and point of contact with phone numbers. Past Performance elements to be reviewed include quality, delivery, cost control, and customer service.
