There will be no minimum guarantees FOB: Destination MILSTRIP: N0018307RQHP101 PURCHASE REQUEST NUMBER: N0018307RQHP101 NET AMT CLAUSES INCORPORATED BY REFERENCE 52.219-22 Small Disadvantaged Business Status OCT 1999 Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT STATEMENT OF WORK SLEEP MEDICINE PHYSICIAN 1.0 SCOPE OF WORK 1.1 The contractor shall normally provide services limited to the interpretation of sleep studies. This is to be done on a part-time basis at a time mutually agreeable between the contracted physician and the Medical Director of the Sleep Lab at Portsmouth Naval Hospital and may be done at times outside of normal business hours and on weekends if so desired. 1.2 Services shall be provided at Naval Medical Center, Portsmouth, Va. (NANMEDCEN). 1.3 Naval Medical Center, Portsmouth shall determine the number of studies to be interpreted on a monthly basis. 1.4 Vendor shall bill for actual work completed. Vendor will be allowed to bill a maximum of 2 times per month. Resulting order will be issued as a not to exceed requirement with no minimum guarantees of work. 1.5 Contractor shall interpret studies utilizing equipment of the Boice Sleep Disorder Laboratory @ Naval Medical Center, Portsmouth, VA. ( Compu-Medics, Nexus and Profusion III). 2.0 OBJECTIVE The purpose of this solicitation is to obtain accurate and timely interpretation of sleep studies for Naval Medical Center Portsmouth, Va.. This contract shall be for a period of six months. 3.0 SPECIFIC DUTIES AND RESPONSIBILITIES The contractor shall interpret/read sleep medicine studies in support of the Pulmonary department using government furnished supplies, facilities and equipment within the assigned unit of the Medical Treatment Facility (MTF). The contractor?s productivity is expected to be comparable with that of other individuals performing similar services. The contractor shall perform the following duties: 3.1. Administrative and Training Requirements. The contractor shall perform limited administrative duties which include maintaining statistical records of clinical workload, participating in education programs, and participating in clinical staff quality assurance functions. 3.2. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. 3.3. Attend Composite Health Care System (CHCS)/Armed Forces Health Longitudinal Technology Application (AHLTA) training provided by the Government for a minimum of four (4) hours, and up to a maximum of 40 hours, if this training is not already possessed by the physician. 3.4. Demonstrate awareness and sensitivity to patient/significant others' rights, as identified within the institution. 3.5. Demonstrate awareness of legal issues in all aspects of patient care and unit function and strive to manage situations in a reduced risk manner. 3.6. Participate in peer review and performance improvement activities. 3.7. Maintain an awareness of responsibility and accountability for own professional practice. 3.8. Participate in continuing education to meet own professional growth. 3.9. Possess and maintain current certification in American Heart Association Basic Life Support (BLS) for Health Care Providers; American Heart Association Health Care Provider course; American Red Cross CPR (Cardio Pulmonary Resuscitation) for the Professional Rescuer; or an equivalent MTF course. Health care workers, not currently in possession of current certification, must acquire certification prior to initiating contract performance. Web based classes do not meet these standards. A copy of the BLS instruction may be obtained from the World Wide Web at: http://navymedicine.med.navy.mil/Files/Media/directives/1500-15a.pdf. 3.10. Comply with the standards of the Joint Commission, applicable provisions of law and the rules and regulations of any and all Governmental authorities pertaining to licensure and regulation of health care personnel and medical treatment facilities, the regulations and standards of medical practice of the MTF and the bylaws of the hospital's medical staff. Adhere to and comply with all Department of the Navy, Bureau of Medicine and Surgery and local Clinic instructions and notices that may be in effect during the term of the contract. 3.11. Comply with the HIPAA (Health Insurance Portability and Accountability Act) privacy and security policies of the treatment facility. Providers shall obtain/maintain a National Provider Identifier (NPI) in accordance with DOD and MTF policy/instruction. 3.12. Participate in executing the Emergency Preparedness Plan (drills and actual emergencies) as scheduled by the MTF (typically semiannually). A MTF personnel re-call list with personal contact information for all military, civil service and contract employees is required to prepare in advance for an actual emergency. You shall provide personal contact information to the designated supervisor upon commencement of services. Should an emergency occur, you will be contacted with shift information and for accountability purposes. 3.13. Background Investigations. By fulfillment of this position, you will have access to Department of Navy (DON) IT systems and/or perform IT-related duties with varying degrees of independence, privilege and/or ability to access and/or impact sensitive data and information. Additionally you may have contact with patients under the age of 18. Therefore, you shall be subject to Information Technology (IT)/Sensitive Information (SI) security requirements which include national and local background checks and a credit check in accordance with Secretary of Navy (SECNAV) Manual 5510.30, as well as a criminal background check in accordance with the Crime Control Act of 1990. It should be noted that in order to receive access to the DON IT system(s) and the sensitive data necessary to perform the duties for this position, you must be a U.S. citizen. You shall be required to complete the paperwork necessary for the Government to complete the background investigations. 3.14 Clinical Responsibilities: Provide Sleep Medicine services, specifically sleep medicine studies interpretation. Additionally, the health care worker shall apply for, and perform, those privileges consistent with the health care worker?s specialty and as outlined in BUMED Credentialing Instruction 6320.66E and granted by the MTF Commanding Officer. The health care worker shall provide interpretation of sleep studies including polysomnograms, multiple sleep latency tests, actigraphy, CPAP titration studies and split-night polysomnograms. The health care worker will provide diagnosis and suggestions for management of sleep-related clinical conditions, including circadian rhythm disorders., insomnias, disorders of excessive sleepiness (e.g. narcolepsy), sleep related breathing disorders (such as obstructive sleep apnea), parasomnias, circadian rhythm disorders, sleep related movement disorders and other conditions pertaining to the sleep-wake cycle. 3.15.Credentialing Requirements: All sleep medicine physicians providing services under this contract shall be subject to the Credentialing Review & Privileging Program as specified in BUMED 6320.66 and in NAVMEDCEN 6320.66 series. The contractor shall adhere to the requirements of any changes made to the aforementioned documents during the term of the contract. The contractor is responsible for performing credentials verification on all healthcare practioners prior to submission of the credentials to the clinic. The contractor shall submit complete credentialing packages to the COR not less than thirty (30) calendar days prior to performance of services. NAVMEDCEN will respond to each credentialing request within thirty (30) days of receipt. The failure of the contractor to submit a completed credentials package will invalidate the requirement for the Navy to respond within thirty (30) days. The failure of the contractor to submit complete credentials thirty days (300 prior to contract start will not constitute an excusable delay or lapse in contract performance. NAVMEDCEN will retain the credentials documentation submitted for each Healthcare practitioner within an official credentials file in accordance with BUMED and NAVMEDCEN instructions. The contractor will ensure that all documentation necessary to keep each individual healthcare practitioner?s file current is submitted to the COR for inclusion in each file. If individual clinical privileges have been summarily suspended pending an investigation into questions of professional ethics or conduct, your performance under this contract may be suspended until clinical privileges are reinstated. No reimbursement shall be made and no other compensation shall accrue to you so long as performance is suspended. The denial, suspension, limitation, or revocation of clinical privileges based upon practitioner impairment or misconduct shall be reported to the appropriate authorities. 3 MINIMUM PERSONNEL REQUIREMENTS 4.11. Possess a Doctorate Degree in Medicine from an accredited institution approved by the Liaison Committee on Medical Education and Hospitals of the American Medical Association, a Doctorate Degree in Osteopathy from a college accredited by the American Osteopathic Association, or permanent certification by the Educational Commission for Foreign Medical Graduates (ECFMG). 4.2. Have graduated from a residency training program in Neurology or residency training in Internal Medicine followed by a fellowship in Pulmonary Medicine approved by the Accreditation Council for Graduate Medical Education or the Committee on Postdoctoral Training of the American Osteopathic Association or those Canadian training programs approved by the Royal College of Physicians and Surgeons of Canada or other appropriate Canadian medical authority. Possess current board certification in either Neurology or Pulmonary Medicine. 4.3. Possess current board certification in Sleep Medicine as determined by the American Board of Sleep Medicine. 4.4. Possess 3 years of experience providing diagnosis and management of sleep-related clinical conditions. 4.5. Possess a current, unrestricted license to practice medicine in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands. 4.6. Represent an acceptable malpractice risk to the Navy. 4.7. Possess U.S. citizenship which is necessary to gain access to DON IT systems and sensitive information (see Section C, paragraph 1.18). 4.8. If awarded a contract, you will be required to obtain a physical examination and immunizations at your own expense prior to initiation of contract performance. The physician must complete the immunization and health examination form provided as Attachment VI. 4.9 Insurance: The contractor shall be solely responsible for any and all liability caused by the acts or omissions of its agents or employees. The contractor shall provide a completed Malpractice Insurance Certification Statement as part of the Quality Control Plan and shall provide an update statement to the COR once a year thereafter during the term of the contract. 4 NON-PERSONAL SERVICE STATEMENT Contractor employees performing services under this order will be controlled, directed and supervised at all times by management personnel of the contractor. Contractor management will ensure that employees properly comply with the performance work standards outlined in the Statement of Work. Contractor employees will perform their duties independent of, and without the supervision of any Government official. The tasks, duties and responsibilities set forth in the task order may not be interpreted or implanted in any manner that results in any contractor employee creating or modifying federal policy, obligating the appropriated funds of the United States Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee, or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and will perform the inspection and acceptance of the completed work.
