- This a combined solicitation/synopsis is for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-07-T-0260s issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 339112: 500 Quotations are due on or before 24 July 2007, 10:00AM Pacific Standard Time to be considered responsive. CLIN 0001 Arthrocare Evac 70 Xtra EIC5872-01 QTY: 120 Each $_____ CLIN 0002 Arthrocare Reflex Ultra 45 EIC4845-01 QTY: 200 Each $_____ CLINs 0001 and 0002 are used for effective removal of both tonsils and adenoids that provides patients with minimal pain and fast recovery. CLIN 0003 Arthroscopic instrument tray (includes: QTY: 3 Each $_____ Marlin straight 12-6000, Barracuda 1.3MM UP 12-3001, Barracuda 1.3MM Straight 12-3010, Blunt Nose Straight 12-5002, Piranha Rongeur 12-4000, Beluga Right 12-5011, Beluga Left 12-5012, Hammerhead Punch 12-8001, Manta 1 Right 12-7009, Jaws Tiger Shark 14-2001, Manta 1 UP 15 12-7001, Manta 1 Left 12-7008, Manta 1 Straight 12-7006, Megamouth 14-2007, Hammerhead Punch L-12-8000) CLIN 0003 is used for all arthroscopic scope cases. Each instrument is used to debride and cut, using different angles while arthroscopically in the patient. If quoting comparable items, please include the specifications for those items.
