Description
DESCRIPTION/SPECIFICATIONS/WORK STATEMENT HOME HEALTH AIDE SERVICES Provide services necessary for satisfactory home health care to VA beneficiaries specifically authorized by this medical center. These services shall include, but are not limited to: Prepare and assist clients with regular and special diets, serving and/or feeding. Provide and assist with care of teeth and mouth. Provide and assist with attire and grooming, including manicure and pedicure. Provide and assist with bathing(bed,tub and shower) Provide and Assisi patient on and off bedpan,toilet or commode. Provide and assist with transfers from bed to chairs and back again, Arrange schedule so that patient follows medical recommendations as increased physical activity. Assurance of patient safety per plan of care. Provide and assist patient with use of assistive devices associated with activities of daily living. Provide and assist with skin care, per a plan of care Supervice patient while patient self-administers medications(OTC or Rx) by reminding patient to take their own medications per plan of care. Measure intake and output and record plan of care. Identify;Implement infection control per plan of care. Other duties as determined and assigned by plan of care. Provide and assist ambulating, and up and down stairs, inside and outside of the home. IT IS MANDATORY THAT ALL NOTES BE LEGIBLE-AND MUST BE RETURNED WITH THE INVOICE IF NOT. HOMEMAKER SERVICES Provide services necessary for satisfactory homemaker services to VA beneficiaries specifically authorized by this medical center. Such services shall include but are not limited to Provide household tasks essential for patient health such as; Light housekeeping as sweeping,mopping dusting, make and change beds, Dishwashing.Tidying kitchen and bedroom. Grocery shopping. Patients personal laundry, Personal laundry,iron and mending. Tidying bathroom. Empty trash. Light meal preparation. Errands. Help patient relearn household skills as part of discharge planning,per plan of care.
