- Provide Speech Pathologist services at the VA Salt Lake City Health Care System. This contract will be for back-up services for approximately 15 days per year. The contractor will be required to respond to consults within 72 hours due to the emergent nature of the problems. The contractor will be required to be credentialed and privileged at the VA Salt Lake City Health Care System. A background investigation will be required and must be completed prior to entering data into the computer system. Speech Pathologists that work under this contract must have a certification by the American Speech Language, Hearing Association & adherence to practice standards set by ASHA, State licensure in speech-language pathology and must be credentialed and privileged and educational requirements of the VA Salt Lake City Health Care System. The contract will run from Date of award through August 31, 2007 with four one-year options to extend the term of the contract. The issue date will be on July 18, 2007 - BOD date is August 3, 2007 - The solicitation will be issued as a commercial item procurement in accordance with part 12 and in accordance with Far Part 15, Contracting by negotiation.
