- This is a Request for Quotations. Quotes are being requested and a written solicitation will not be issued. Requirement number 770-7-048310 is issued as a Request for Quotations. The Centers for Medicare and Medicaid Services (CMS), four Regional Offices: Dallas, Chicago, Denver, and Kansas City has a requirement for a contractor to provide Laserfiche upgrades and software maintenance renewals for one year. The period of performance will be August 18, 2007 through August 17, 2008. This is a firm fixed price procurement using simplified acquisition procedures. This procurement is being set aside specifically for small business concerns. The North American Industry Classification System Code (NAICS) is 423430. Expressions of interest must be emailed to phillip.harrell@cms.hhs.gov no later than 3:00pm on July 18, 2007. Responses may be sent electronically in MS Word format to phillip.harrell@cms.hhs.gov or in paper no later than 3:00pm on July 26, 2006 to Phillip Harrell, Department of Health and Human Services, Centers for Medicare and Medicaid Services, Office of Acquisition and Grants Management, 7500 Security Blvd, C2-21-15, Baltimore, MD 21244. Responses shall be no more than 10 pages and shall include the following: 1) Contractors capabilities overview; 2) Past performance information (relevant experience to perform the required services); and 3) Price of the deliverables. All responses will be evaluated on a best value basis using the following evaluation criteria: 1) Technical Approach 2) Past performance and 3) Prices.
