Description
The Oklahoma National Guard is to provide job related medical surveillance examinations to approximately 217 OKNG full-time support (FTS) who are potentially exposed to health hazards in the work environment. The Contractor (Ktr) will provide all la bor, supervision, doctors, tools, equipment, mobile unit, supplies and incidentals to perform the mobile surveillance services for the OKNG. The Ktr shall ensure all procedures are conducted in accordance with applicable American Board of Occupational and Environmental Medicine (ABOEM), American Association of Occupational Health Nurses (AAOHN), U.S. Department of Labor, Occupational Safety and Health Administration (OSHA), and the National Institute for Occupational Safety and Health (NIOSH) Standards. T he Ktr shall ensure all clinical laboratory services are performed by a laboratory licenses through the U.S. Department of Health and Human Services (DHHS), Health Care Financing Administration pursuant to the terms of the Clinical Laboratories Improvement Act of 1967 (42 U.S.C. 263a) and the College of American Pathologies. The Ktr shall provide a Board eligible or a Certified Occupational Medicine Physician to conduct job-related medical surveillance physical examination and occupational health consultat ion services. The Ktr shall arrange for the opportunity to consult with a representative of the OKNG by telephone to coordinate or resolve issues that might arise before, during, and after the examination period. The Ktr shall perform a comprehensive rev iew and evaluation within 30-days following the conclusion of the examinations. The Contracting Officer's Representative (COR) will contact and schedule a follow-up review and evaluation meeting to discuss positive and negative aspects of the examinations . The Ktr shall ensure licensed medical technicians will perform the Medical Surveillance Examination elements. The medical surveillance person(s) to operate the audiogram must be certified by the Council for Accreditation in Occupational Hearing Conserv ation (CAOHC). Personnel conducting Spirometry shall be certified through a program approved by the National Institute for Occupational Safety and Health (NIOSH). The Ktr shall ensure instrumentation must meet or exceed appropriate performance standards and accuracy must be verified in accordance with current Occupational Medicine Certification standards. The Ktr shall provide the COR with a Quality Assurance Medical Surveillance Plan listing the procedures or processes, which shall be utilized to meet t he requirements discussed in the statement of work. Medical Surveillance protocol shall consist of a physical examination to include the following: a complete occupational health history, hands on physical by a board eligible or board certified doctor, OS HA Respirator Questionnaire with Clearance that shall medically clear employees to wear a respirator, hearing conservation program with results reported to DOEHRS-HC, pulmonary function test measures lung capacity for respirator clearance, height weight an d blood pressure which is required for the respirator clearance, vision screen near and far acuity color and vertical/lateral testing, blood chemistry baseline for liver function, lipid profile which includes HDL and LDL total cholesterol glucose and blood sugar levels to establish potential diabetics blood lead with zinc protoporphytrin, complete blood count tests for red and white blood cell count and hematology capability, urine dip test for sugar levels protein and blood in urine, qualitative fit test f or 1/2 face respirators, chest x-ray with b-reader, and employee 40-years of age and over shall receive a Prostate Specific Antigen test.
