- This amendment is for 1. Additional information for scope of work 2. Add evaluation criteria. 3. Add Period of Performance Evaluation: Offers will be evaluated using the evaluation criteria outlined in FAR 13. The award will be made to the vendor whose proposal is most advantageous to the Government. The evaluated non-cost factors for this acquisition shall be Past Performance . The Government will evaluate the extent and quality of the offerors experience performing comparable services. The vendor shall provide at a minimum three (3) references which shall include a brief description, date of award, award amount and a point of contact with a telephone number and an email address. Each vendor will receive an acceptable/non acceptable pass performance rating. All vendors which receive an acceptable past performance rating will be considered for award. Upon completion of past per formance evaluation, price will become the determining factor. Vendors shall provide a firm fixed price quote for the services. Period of Performance: Period of Performance shall be from date of award to September 28, 2007 Scope: The contractor shall provide network services to rebuild the domain network server. The contractor shall provide network configuration for Moncrief Army Community Hospital (MACH) located at Fort Jackson, South Carolina for the period from date of award through September 28, 2007. Network configuration services and repair may be performed in MACH or any of the buildings assigned to MACH, Task Force East, Wier Tower, McCrady, Dental Activity, Physical Exam Station (PES), or Veterinary Service. The contractor will provide all technical automation support to include but not limited to T1 connectivity, network scans, software pushes for upgrades, CHCS and AHLTA access, Blackberry support, user accounts/passwords, Outlook e-mail management, and gene ral troubleshooting for network assigned computers. In order to meet the workload requirement, at least one technician should be provided at all regularly scheduled duty times during the course of this contract. The major function and requirement of the contractor provided technicians is to troubleshoot and fix fiber links, hardware, and software problems. The contractor will be responsible for performing work that involves the deployment of new systems to include transferring data, joining computers to the domain, setting up user profiles. Tasks include formatting hard drives and installing the operating sy stem, user software, and remote access software and data files. Additionally the contractor will be required to fill out and maintain government forms for property accountability and track all work using the HEAT helpdesk software. These services are needed to maintain the backbone of the hospital computer network system which currently consists of 1,000 networked computers as well as printers and other peripheral devices. Normal Hours of Operation: 0730 to 1630, Monday-Friday.
