Description
THE US GOVERNMENT INTENDS TO ESTABLISH AN PURCHASE ORDER FOR: Non Personal Service To Rent 2 Each Sanitary Container Each container consists of: 2Each Toilets, 4 Each Washbowls and 2 Each Urinals Please provide prices for Freight and Installation with your Quote! NOTE: ONLY CCR AND ORCA REGISTERED VENDOR'S ARE ELEGIBLE TO PROVIDE A QUOTE!!!!!!!!!!!!!! SCOPE OF WORK DOCUMENT # WK4F5E-7114-1006 The contractor is to provide all labor and supervision to meet the requirements of this performance based work statement to support the Latrine at USAMMCE, Bldg # 4155, Pirmasens. Period of performance shall be in accordance with the schedule set forth in this requirement(s). All facilities and support facilities must be in place ASAP. Provide a copy of an employee list of Host Nation employees, to the Task Force S-2 and U.S. military police as soon as possible, but not later than (Date). Contractor(s) are to have background checks conducted and provide badges for all local hires and necessary contract personnel working on the contract. All Contractor personnel shall maintain identification badges on their person at all times while in the area of operation. Contractor shall provide all supplies and services required to accomplish the mission. This includes, but is not limited to water requirements and sink facilities. Removal of any grey water produced as a result sink facilities shall be the responsibility of the contractor. Contractor shall return all grounds to their original condition upon completion of mission. This includes , but is not limited to removal of all structures, equipment, power lines, cords, wires, trash and lumber. Additional requirements and modifications may be required in an urgent and compelling manner to ensure the success of operations. All coordination of these requirements shall be conducted through the Contracting Officer. Contractor Furnished Property, Facilities and Services: Contractor shall provide all materials and equipment (unless to be provided by the Government in accordance with Government Furnished Property, Facilities and Services), personnel and management to support all requirements stated in the Work Statement. The Contractor will provide all labor, material, transportation and equipment required for the delivery, installation, removal of sludge to the appropriate sewage disposal pint, servicing, and pick up of the latrine facilities at the expiration of the rental period. Toilet facilities are to be of the standard commercial based units. All facilities shall provide hand cleaning agents (dispensers) or fully functional hand washing areas, toilet paper holders, clothing hooks, ventilation outlets, lockable doors and heating. Contractor is responsible for inspection and maintenance to ensure all facilities are free of cracks and leaks and are in good operation order. All facilities not meeting these standards shall be replaced or deficiencies need to be corrected within 6 hours of notification. Location for the latrines shall be coordinated with the Contracting Officer Representative (COR) at site. Disposal of all waste material shall be in accordance with host nation regulations. All spills inside and outside the area of operation during the removal of sewage shall be cleaned up immediately in accordance with local laws and regulations. Cleaning, toilet paper, and towels are not required. The gray water pipe is about 10 m to the container. Sewage is about the same distance. SERVICES: Sufficient hot and cold water capacity to meet personal hygiene requirements. Facilities will be lighted, heated, and have adequate ventilation. Facility spacious enough to provide change area with towel hooks. Any maintenance or services required completed within 6 hours notification. Location for toilet: U.S. Army Medical Materiel Center Europe, Husterhoehkaserne, Pirmasens, Bldg # 4155, in front of existing toilet facility. REQUIRED PERIOD OF PERFORMANCE: 15 JULY 2007 TO 15 JUL 2008 YOUR QUOTE IS REQUIRED NOT LATER THAN 05 JULY 2007, 14.00 HRS CENTRAL EUROPEN STANDARD TIME!!!!!
