- The MEDCOM Health Care Acquisition Activity, Center for Health Care Contracting, Fort Sam Houston, Texas, intends to award a Single Award, Personal Services contract to provide (12) Community-Based Health Care Organization (CBHCO) Licensed Clinical S ocial Workers, to perform services within the Continental United States (CONUS) as part of the Army wide Behavioral Health (BH) support of Soldiers and Families at CBHCO Army medical clinics located in Boston, MA; Redstone Arsenal, AL; Little Rock, AR; Sal t Lake, UT; Virginia Beach, VA; Rock Island, IL; Orlando, FL and Sacramento, CA; and (35) Licensed Clinical Social Workers at the Primary Care Clinics Military Treatment Facilities: Fort Bliss, Fort Bragg, Fort Carson, Fort Drum, Fort Hood, Fort Knox, Fort Riley, Fort Benning, Fort Campbell, Fort Gordon, Fort Stewart, Fort Lewis, Brooke Army Medical Center; Fort Sam Houston, Texas, and Walter Reed Army Medical Center; Washington, DC. Contractor shall provide clinical assessments, treatment, case management referral, and education according to their professional discipline, Army Medical Department standards, and the DOD directed Post-Deployment Health Evaluation and Management (PDH-CPG). The contractor shall provide these services as part of the multidiscipl inary health care team to patients who are eligible beneficiaries of the Medical Health Care System. Contract service providers will also provide BH screening, assessment, and direct care, specialty referral and longitudinal BH care management to deployed service members and their families with conditions and concerns related to their deployment. The proposed acquisition is a competitive small business set aside under NAICS code 621330 Offices of Mental Health Practitioners (except Physicians) and FSC code Q201 General Health Care Services. Prospective offerors in order to receive an award must be registered with the Central Contractor Registration Program. The contract period will be for a base period from 18 October 2007 through 17 October 2008 and four 1-year option periods through 17 October 2012. The solicitation is expected to be released on or about 09 July 2007 and will be available for downloading at https://www.fedbizopps.gov/.
