Description

The Contracting Department, Fleet and Industrial Supply Center (FISC), Norfolk, VA 23511-3392 intends to negotiate a sole source Firm Fixed Price Contract and will utilize procedures under FAR Subpart 13.5, Test Program for Certain Commercial Items, with Intralase Corporation, 9701 Jeronimo Rd, Irvine, CA 92618-2020, to procure drop shipments of Patient Interface Kits and for maintenance services in support of the Intralase Femtosecond Laser Model 2 System at the Naval Hospital Camp Lejeune Ophthalmology Department, Camp Lejeune, North Carolina. The Patient Interface kits were specifically designed for use with the Femtosecond Laser system manufactured by Intralase Corp. All pertinent data, software, and drawings required to manufacture the Patient Interface kits are proprietary to Intralase Corp. The delivery date for the resultant contract will be 01 Oct 2007 through 30 Sep 2008 with provisions for four (4) one (1) year option periods. The complete RFP package will be available on or about 19 July 2007. FSC Code is 6515 and the NAICS is 339112. Potential offerors are hereby notified that the solicitation and any subsequent amendments to the solicitation will only be available by downloading the documents at the FEDBIZOPPS website at http://www.fedbizopps.gov/ or the NECO website at https://www.neco.navy.mil/ as this office no longer issues hard copy solicitations. By submitting a proposal, the vendor will be self-certifying that neither the requestor or the principal corporate officials and owners are currently suspended or debarred, or otherwise ineligible to receive contracts from any Federal agency. Failure to do so may be grounds for refusing to accept the proposal. NOTE 22 APPLIES.
