- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quote (RFQ). Submit written offers (oral offers will not be accepted), on Reference Number F4T147051A2. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-17 and Defense Acquisition Circular 91-13. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). North American Industrial Classification Standard 423910 applies to this procurement, and the small business size standard is 100 employees. The following commercial items are requested in this solicitation: CLIN 0001: Gametime Volcano Climber, model number 7052, 8? Volcano Climbers-Sandstone, or equal, 1 each: CLIN 0002: Gametime Prime Time Modular Unit, Model RDU, Tree Play Palette, or equal, 1 each: CLIN 0003: Installation of Line items 1 & 2, 1 each. CLIN 0004: Sub Base, Provide and Install 1920 square feet concrete sub base at 4? depth, light brown finish concrete with wire mesh reinforcement. Tapered down edge, 1 each: CLIN 0005: Provide and Install 1920 square feet of poured in place surfacing- with 1515 square feet @ 2? depth and 405 square feet @ 3? depth, surfacing will be at 3? depth and will slope down to 2? depth, 1 each: Also, contractor is responsible for site. Must be level (less 2% grade), free of obstruction, and accessible. Location of utilities, bonds, permits, site preparation, and security not included. Must comply with Consumer Product Safety Commission Guidelines. Must Fit into 54? x 54? space. CLIN 0006: Freight; 1 each. Note: DO NOT SHIP UNTIL AFTER 1 JULY AND NLT 15 JULY 2007. Items should be delivered 30 Days ADC, FOB: Destination for delivery to Whiteman AFB, MO 65305. The following provisions and clauses apply to this acquisition: FAR 52.211-6, Brand Name or Equal; FAR 52.212-1, Instructions to Offerors--Commercial Items. Quotes will be evaluated and awarded to the offeror with the lowest price technically acceptable; FAR 52.212-4, Contract Terms and Conditions ? Commercial Items; FAR 52.252-2, Clauses Incorporated by Reference; and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Order ? Commercial Items. Offerors shall include a completed copy of FAR provision 52.212-3 and DFARS Provision 252.212-7000 with their quote (Note: On-line Representations and Certifications Application (ORCA) has been implemented in FAR Clause 52.212-3 effective 01 Jan 05. ORCA enambles vendors to submit and update their representations and certifications on-line. ORCA is part of the Business Partner Network (BPN) and is located at http://orca.bpn.gov. AFFARS 5352.215-9001 NOTICE OF PRE-BID/PRE-PROPOSAL CONFERENCE (MAY 1996), (a) A pre-bid/pre-proposal conference will be conducted at 509th Contracting Squadron on 20 June at 9 a.m. for the purpose of answering questions regarding this solicitation, (b) Submit the names of all attendees (not to exceed 2) to SSGT Michael Johnson NLT 14 June. This information must be provided in advance in order to ensure access to the military base/conference site and adequate seating for the conference attendees, (c) Bidders/Offerors are requested to submit questions to the point of contact noted above not later than 22 June. Information provided at this conference shall not qualify the terms and conditions of the solicitation and specifications. Terms of the solicitation and specifications remain unchanged unless the solicitation is amended in writing. If an amendment is issued, normal procedures relating to the acknowledgment and receipt of solicitation amendments shall apply, (d) A record of the conference shall be made and furnished to all prospective bidders/offerors. The record should include minutes of the meeting, including questions (on a non-attribution basis) and answers. The Government reserves the right to award on a multiple award or an all or none basis. All firms must be registered in the Central Contractor Registration database @ www.ccr.gov to be considered for award.
