Description
This is a combined synopsis/solicitation for a fixed price contract - commercial items prepared in accordance with FAR 12.6 using Simplified Acquisition Procedures under the test program for commercial items found at FAR 13.5, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. No other solicitation will be issued. This solicitation identified as RFP 07T677012 includes all applicable provisions and clauses in effect through FAR FAC 2005-16 (March 2007). The North American Industry Classification Code is 923120. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. This is a total small business set-aside. Purpose of the contract To develop the Indian Health Service Health Promotion/Disease Prevention community assessment toolkit to increase accessibility; update the HP/DP website; and assist with special prevention projects. Background and need Health Promotion/Disease Prevention is one of the three IHS Director??s Initiatives to reduce health disparities among American Indian/Alaska Native populations. Many of the health conditions and chronic diseases that are related to lifestyles issues such as obesity, physical inactivity, poor diet, substance abuse, and injuries. To help meet these challenges, the Indian Health Service (IHS) has launched a Health Promotion and Disease Prevention (HP/DP) Initiative to develop a coordinated and systematic approach to enhance preventive health approaches at the local, regional, and national levels. One of the important components of this national effort is to create a one stop clearinghouse for IHS/Tribal/Urban programs to access community assessment tools, health profiles, and promising practices/local efforts that are culturally acceptable and sustainable. The lack of community health assessment guidelines hinders efforts to determine whether preventive and curative health care programs are being effective in their ultimate goal of creating a healthier community. Indian Health Service Health Promotion/Disease Prevention Program Specialist Scope of Work The overall goal of the project is to research and develop community assessment tool and assist with the development of a community assessment guide book. Research community and clinical health promotion and disease prevention (HP/DP) best and promising practices, resources, and training that are appropriate for American Indian/Alaska Native communities. The contractor will gathers, assesses, updates and enters information on prevention resources including on-going collecting, assessing, and updating entry of information from a variety of sources including published public health data, subject matter experts, and community programs. Contractor will work closely with the IHS Information Technology staff to put the community assessment tools at the HP/DP website, update, and archive outdated information. Scope of Work ?? The contractor will research and develop community assessment tool and draft a ??how to?? conduct community assessment guide with input from the HP/DP Community Assessment Working Group and refine as recommended. ?? Contractor shall establish, implement and maintain an electronic database to increase access to information about resources, training and best and promising practices, local efforts, and community assessment tools that are effective in American Indian and Alaska Native communities. The contractor will work closely with IHS IT staff to ensure information are updated and outdated information be sent to archived. Skill in applying complex fact finding, analytical and problem solving methods and techniques, knowledge of and evidence-based interventions. ?? Gathers, assesses, updates and enters information on prevention resources, training, and prevention and interventions including on-going collecting, assessing, and updating entry of information from a variety of sources including published data, subject matter experts, and community programs. Contractor must posses a working knowledge and ability, with a high degree of resourcefulness and ingenuity, to apply a wide range of public health principles, theories, and processes related to collecting and assessing best and promising preventive practices. ?? The contractor will work closely with IHS IT staff to ensure that the community assessment tools, resource, training, and best practice information on the HP/DP website is accurate and up-to-date. Organizes and presents resource information in a practical way to be understood by individuals who have limited skills with computer software or technologies. Contractor must posses a working knowledge and skills in applying a wide range of methods used to gather, analyze, and evaluate information, draw conclusions, recommend appropriate action and ability to search and identify best and promising practices appropriate for Indian communities. ?? Reviews and posts information about grant resources, best and promising practices that are available for IHS, Tribal, Urban communities, and conduct follow-up with community contacts to determine their experience and satisfaction with granting agencies and foundations. ?? Works closely with Area Health Promotion Coordinators to insure that IHS Areas are aware of prevention efforts throughout their region. Assist Area Coordinator with other health promotion and disease prevention projects. ?? Research and coordinate a group of pilot communities to further refine community health assessment instruments and support tools to ensure population appropriateness. Working knowledge and experience with developing training and tools for community health assessment. Special program clearances or approvals (If applicable to the acquisition) ?X Access IHS HP/DP website to update best and promising practices website ?X Email address to interface with IHS Area Coordinators and other groups Estimated Performance 3 months (40 hours work week) Evaluation Criteria The contractor must have documentation of each of the evaluation criteria listed below. ?? Contractor is knowledgeable of community assessment tools that are relevant to American Indian/Alaska Native population. Posses the ability to modify community assessment instrument to ensure cultural appropriateness. (15 points) ?? Contractor is knowledgeable of public health concept, theories, and interventions that are relevant to American Indian/Alaska Native population. Contractor has the ability to research and contact various organizations, agencies, and departments to obtain current resources, training, and best and promising practice information. (15 points). ?? Contractor is knowledgeable of computer software program including excel, access, and Microsoft Word. (15 points) ?? Contractor is knowledgeable of and skills in applying a wide range of methods to gather, analyze, and evaluate information, draw conclusions, and recommend appropriate action. (15 points). ?? Contractor has the ability to formulate focus group questions and conduct focus groups, design survey instruments and summarize results. (10 points) ?? Contractor is knowledgeable of reliable and valid health survey instruments that have been utilized in American Indian/Alaska Native population. (10) ?? Skill in written and oral communication, ability to facilitate discussion, present findings, and recommendations, and promote or implement new ideas. (10) ?? Contractor must posses a working knowledge of the Indian health care system, and the operations of the IHS, Tribal and Urban programs. (10 points) Schedule of deliverables/reporting requirements -Contractor will research and draft community assessment tools for the working group to review. Monthly -Contractor will revise and update a ??how to?? conduct community assessment guide for the working group to review. Weekly -Provide recommendations with written plan for further refinement of the HP/DP website on best practices, resources, and training. Monthly -Contractor will research and contact various organizations, agencies, and departments to ensure current resources, training and best practice information are up-to-date on the HP/DP web site each month. Monthly -Contractor will work closely with the HP/DP Coordinator and Information Technology to develop community assessment tools and health indicators to meet the needs of IHS HP program. Weekly -Contractor will schedule and facilitate the community assessment working group, prepare minutes reflecting discussion, action, and recommendations. Monthly -Contractor will communicate with each of the IHS Area Health Promotion/Disease Prevention Coordinators to receive up-to-date training information to ensure the web site is up-to-date each month. Monthly -Contractor will pilot and refined community health assessment instrument and pilot with a group of communities by January 2007. Monthly -Contractor will assist the HQ HP/DP coordinator with other projects as assigned. Weekly QUESTION DEADLINE:.
