Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W45XTR73241053 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This acquisition is solicited as a 100% Small Business set-aside for NAICS 325413, size 500 employees. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand name or Equal applies to this acquisition. The requirements of this solic itation are: Item no. 0001: Non Personal services: RAPFFN 26 Cassette TLIIQ System Analyzer/printer/Qcette to be provided at no cost to the government and purchase of consumables to run Fetal fibronectin testing; quantities to be delivered as indicated on each subline item listed. Delivery to: US Army MEDDAC DACH, FT Hood, TX. Base period of performance 1 Oct 07  30 Sep 08. SubCLIN 0001AA: Cat #01200, RAPFFN 26 Cassettes, 16 Kits; Deliver during the first working week of each month as follows: 2 k its in (Oct 07, Jan 08, Apr 08, Jul 08), 1 kit in (Nov 07, Dec 07, Feb 08, Mar 08, May 08, Jun 08, Aug 08, Sep 08). Total: 16 kits. SubCLIN 0001AB: Cat #01166, Rapid FFN Controls, 2 Kits; Deliver during the first working week of each month as follows: 1 kit in (Oct 07 and Apr 08). Total: 2 kits. SubCLIN 0001AC: Cat #00797, Specimen Collection (8s), 60 Kits; Deliver five (5) kits during the first working week of each month for a total of sixty (60) Kits over twelve (12) shipments at no charge. SubC LIN 0001AD: Cat #01175, TLIIQ Qcette, Assem/Pkg/Lbeled, 1 Pkg. Deliver one (1) package in Oct 07 during the first working week of the month for a total of one (1) package over one (1) shipment; SubCLIN 0001AE: Cat# 01088, Printer Labels, 1 Pkg. Deliver one (1) package in Oct 07, during the first working week of the month for a total of one (1) package over one (1) shipment. Contract Line Items no. 1001-1001AE are for the 1st option period 1 Oct 08  30 Sep 09, items and quantities as stated in the base period. Contract Line Items no. 2001-2001AE are for the 2nd option period 1 Oct 09  30 Sep 10, items and quantities as stated in the base period. Contract Line Items no. 3001-3001AE are for the 3rd option period 1 Oct 10  30 Sep 11, items and quantit ies as stated in the base period. Contract Line Items no. 4001-4001AE are for the 4th option period 1 Oct 11  30 Sep 12, items and quantities as stated in the base period. Provide pricing for the base and option years. The manufacturer for these items is Adeza Medical Corporation; all items are Brand name or equal specifications. Salient Characteristics: CAT #01202: reusable Quality Control device; QCette verifies analyzer performance within specification; total tech time less than 60 seconds per test; results in 23 minutes. CAT #01200: 26 cassettes; run positive and negative c ontrols; shelf life: 18 months from the date of manufacture. Cat #01166: includes a liquid Positive Control (2.4 ml/Red bottle) and a liquid Negative control (2.4 ml/Blue bottle); expiration date of 6 months once opened. Cat #00797: contains 8 sterile, dacron swabs and 8 tubes with buffer azide solution; one specimen collection kit enough for 8 different patient samples. Cat #01088: Prints TLi IQ patient and QC results; packaged 400/roll. Cat # 01175: stored at room temperature (15 deg to 30 deg C/ 59 deg to 86deg F) lasts indefinitely if stored properly. Preferred shipping method is FOB destination to US Army MEDDAC DACH, Material Branch Log Div, Building 36000, Ft Hood, TX 76544-5063. Vendor shall indicate on their price quote if shipping method will be other than FOB Destination.
