- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442762580545; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-16 and Defense DCN 20070531. This acquisition will be 100% small business set-aside; the North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 employees. OR EQUAL Item 0001: eMax 2 Motor OR EQUAL: 1 each; Part Number: EMAX2 Item 0002: eMax 2 Foot Pedal OR EQUAL: 1 each; Part Number: EMAX2-FP. Item 0003: eMax 2 Console: 1 each; Part Number: SC2000. Item 0004: eMax 2 Sterilization Case/Tray OR EQUAL: 1 each; Part Number: EMAX2-TRAY. Item 0005: Attachment Cleaning Brush OR EQUAL: 1 each; Part Number: ACB. Item 0006: Promotional Literature Binder OR EQUAL: 2 each; Part Number: 10-0015. Item 0007: 5.0cm Short Attachment OR EQUAL: 2 each; Part Number: SHORT. Items 0008: 8cm Medium Attachment OR EQUAL: 1 each; Part Number: MEDIUM. Item 0009: 8cm Angle Attachment OR EQUAL: 2 each; Part Number: QD8. This request consists of a trade-in of the following product: Anspach MicroMax; Model number: 1P-73R7; Serial number: 033-4411-LGE. All equal items must meet or exceed the features of the items described. The delivery location is Travis AFB, California. Ensure you include the delivery time for project completion. Upon award the contractor will be required to coordinate delivery with the customer. The following provisions and/or clauses apply to this acquisition.
