- Request for Information (RFI)/ Sources Sought inquiry into your organizations interest to provide maintenance services to our Fresenius Dialysis Equipment. The VA Eastern Colorado Health Care System (ECHCS ), Denver, CO and VISN 19 Rocky Mountain Network Contracting Office contemplates an award of a Requirements Contract (Firm Fixed Price) for Fresenius Dialysis laboratory equipment services resulting from a formal solicitation. Dependant upon responses, the proposed contract award priority shall be to Service Disabled Veteran Owned Small Businesses. The period of performance for this contract will be one year, with additional four-one year option periods. The contract base year is October 1, 2007 through September 30, 2008. The contractor shall provide all labor, parts, equipment, transportation and supervision necessary to provide repair and preventive maintenance of Dialysis Equipment located at the ECHCS. The Fresenius Dialysis equipment for maintenance services is as follows: Quarterly - Fresenius Dialysis Delivery Unit, Model A2008H: - 7H0SN770 to include CMS208K Monitor & Keyboard, 7H0SN616 BTM Blood Pressure Monitors Semi-Annual - Fresenius Dialysis Delivery Units, Model A2008K: - 3K0S E797, 3K0S E798, 3K0S E811, 3K0S E816, 3K0S E818, 3K0S E819, 3K0S E823, 3K0S E825, 3K0S E907, 3K0S E936, 3K0S E937, 3K0S F221, 3K0S F232 and 3K0S F242; and Millenium Reverse Osmosis (RO) Water Treatment Units: 17178, 1117624 and 1117632. Annual - Fresenius Model FDS08 Computer System. The ECHCS shall not provide service manuals or service diagnostic software to the contractor. The contractor shall obtain, have on file, and make available to its Field Service Engineer all operational and technical documentation, (such as: operational and service manuals, schematics, and parts lists), which are necessary to meet the performance requirements of the contract. Unauthorized and illegal copies of software will not be used on the ECHCS premises. This will be a full service contract, seven (7) days per week, 24 hours per day, including Federal holidays. All scheduled maintenance service shall be between the hours of 8:00 a.m. until 5:00 p.m., Monday through Friday. Preventative maintenance will be performed quarterly on the A2008H machine, beginning month of October. Preventative maintenance will be performed semi-annually on the A2008K machines and RO units during the months beginning July and January. Annual calibration will be performed during the month of October. Preventive maintenance services will be scheduled in advance and will be predominantly performed on Tuesdays and/or Thursdays. Again, this is only an inquiry, no quote or proposal is requested at this time. Should no inquires be made, the VISN 19 Rocky Mountain Network Contracting Office intends to acquire this maintenance services from Fresenius USA, Inc, under the authority of FAR 6.302 with NAICS Code 811219. As such, if this maintenance services requirement is within your organizations capabilities and interest, please reply back to me with a statement describing your capabilities and Fresenius Equipment Service Certification.
