Description
Overton Brooks VA Medical Center (OBVAMC), Shreveport LA, requires a contractor to provide design services for contract drawings and specifications for Replacement of Building 5 to Expand Specialty Care Clinics at the OBVAMC. The project will completely demolish Building 5 and construct a new facility to house the Eye Clinic, Podiatry Clinic and Home Base Primary Care section. Space criteria for these functions will be provided by the VA. Also included in this project is the temporary relocation of the existing functions in Building 5 to temporary buildings and utility services required to make the temporary building functional. The A/E shall incorporate the requirements of all applicable codes; life safety, ADA, national electric, national plumbing, VA HVAC design criteria, etc. and other applicable VA construction guidelines into the design of this project. The current asbestos assessment shall be reviewed prior to design and incorporate any updated findings to ensure appropriate information is provided in the contract documents. The new building shall be of similar architectural design as to blend into the existing architecture of the facility and the structure shall be capable of expansion to four floors. The solicitation is to be issued as a 100% Service Disabled Veteran Small Business Set-Aside under North American Industry Classification System (NAICS) Code 541310. This size standard is $4.5 Million per year. This information is only used for classification purposes in the representation and certification portion of the Request for Proposal (RFP). NOTICE TO OFFERORS: Authorization to proceed with contracting of this project is subject to the availability of funds. Funds are not presently available for this requirement. No award will be made for this project until funds are available. The government reserves the right to cancel at no cost to the Government. If interested in providing a proposal for this requirement, the attached Standard Form 330, Architect-Engineers Qualifications form shall be completed and provided to this office no later than 3:00 P.M. CST, June 27, 2007. The SF 330 may be submitted electronically to Carolyn.Lee3@va.gov. The completed SF 330 will be evaluated by the Chief Engineer, Overton Brooks VA Medical Center. Engineering Services, Overton Brooks VA Medical Center, shall hold discussions with the top four (4) firms selected based on submittal of SF 330's. Engineering Services shall provide a listing, in order of preference, to A&MMS for negotiations. In accordance with FAR 36.606, negotiations shall be conducted beginning with the most preferred firm in the final selection. Contractors must be registered in the Centralized Contractor Registration (CCR) in order to receive a contract award. Contractors may access the CCR at http://www.ccr.gov. Any offeror submitting a proposal is required by the Federal Acquisition Regulations to use the Online Representation and Certifications Application (ORCA). ORCA is a web-based system that centralizes and standardizes the collection, storage and viewing of many of the FAR required representations and certifications previously found in solicitations. With ORCA you now have the ability to enter and maintain your representation and certification information, at your convenience, via the internet at http://orca.bpn.gov. To register in ORCA you will need to have two items: an active Central Contractor Registration (CCR) record and a Marketing Partner Identification Number (MPIN) identified in your CCR record. Your DUNS number and MPIN act as your company's ID and password into ORCA. The basic information provided in your CCR record is used to pre-populate a number of fields in ORCA. Once in ORCA you will be asked to review pertinent information pre-populated from CCR, provide a point of contact, and answer a questionnaire that contains up to 26 questions. The questionnaire is to help you gather information that you need for your clauses. The questionnaire is not the official version. Be sure to read the provisions carefully. The answers you provide are then automatically entered into the actual FAR provisions. You are required to review your information, as inserted, in contest of the full-text provisions for accuracy; acknowledge three additional read-only provisions, and click a time/date stamp before final submission. You will need to review and/or update your ORCA record when necessary, but at least annually in order to maintain active status.
