- CONTRACTOR SHALL CONTACT Y. WILLIAMS at Yvette.Williams@med.navy.mil TO REQUEST A COMPLETE COPY OF THE RFQ. Contractor shall provide Dental Labatory Services in support of the Naval Dental Center, 2D Dental Battalion, Camp Lejeune, North Carolina, for a base year and four option years in accordance with the attached Statement of Work. STATEMENT OF WORK Selected vendor must be able to provide full contract support immediately upon award of a resulting order. Vendor shall provide quality dental laboratory services as required for patient care. A list of possible required items are identified in Section B of the DD Form 1155 ?Order for Supplies or Services?. Quality dental services shall be provided during normal working hour (8:00 A.M. to 4:30 P.M.) Monday through Friday excluding Federal holidays. 1. General a. The Contractor shall be governed by all laws and regulations within the North Carolina Dental Laboratory Association (NCDL), the National Association of Dental Laboratories (NADL), the Federal, State and Local laws plus any Federal Regulations as applicable to the performance of this contract. b. The Contractor shall not accept any instructions issued by any person employed by the U. S. Government, other than the Contracting Officer (KO) or designated Government Representative from the 2D Dental Battalion/Naval Dental Center, Camp Lejeune (NDC) all acting within the limits of their authority. c. The Contractor shall be qualified, certified and authorized by the NCDL and the NADL in the administration of dental services. Contractor shall maintain certifications throughout the entire contract period. Qualifications must be current and the Government shall maintain the right to seek proof of qualification prior to award and anytime during contract period of performance. 2. Scope of Work a. The Contractor shall provide quality dental laboratory services in accordance with Section B of the DD Form 1155 ?Order for Supplies or Services?. All dental supplies provided shall be certified for use and in ready to use condition. All equipment supplied shall be free from manufacture?s defects and replacement, if required, shall be at the Contractor?s expense. b. The Contractor shall provide telephonic communications with the Government to discuss dental services relating to the performance of this contract. c. The Contractor shall be notified, by phone, when dental laboratory services are required. An authorized Government Representative from the 2D Dental Battalion/ Naval Dental Center, Camp Lejeune (NDC) will notify the Contractor phone. This notification will begin the clock for the required turn-around times. d. Required turn-around times begin with the phone call notifying the Contractor of the needed service, and end when the service is completed and the items are received by NDC. The Contractor shall provide free pick-up and delivery services and shall be responsible for all shipping costs, whether the items are picked up in person, or a carrier/shipping service is used. NDC will allow the Contractor to furnish an account number that can be used by NDC as NDC sees fit for the purpose of shipping the items to the Contractor as provided in the contract. Required turn-around times are as follows and are in terms of working days, excluding federal holidays. ? For normal service, the required turn-around time for single and multiple items is 12 days. ? For expedited service, the required turn-around time for single and multiple items is 7 days. e. The Contractor shall assume all responsibility and remains liable for all items picked up until they are successfully delivered to an authorized Government Representative of NDC. This successful delivery will be represented by a signed receipt from designated Government Representative. Any loss, damage, or misplaced items during the shipping process will be replaced by the Contractor with exact/like items/services at no cost to the Government. f. Once the service has been rendered, and the item received, NDC reserves the right to determine the quality of the service/returned item. Applicable industry standards will be used to assess quality and dependability of lab services provided. If an item/service is determined to be below industry standards for any reason by NDC, the Contractor shall re-work the item and/or fix the problem at no expense to the Government. In this case, the Contractor agrees to provide the re-work service in compliance with the expedited service delivery time-frame mentioned above. g. The Government does not offer any guaranteed minimums of dental services to be provided under this contract. h.
