- The GLAC (Great Lakes Acquisition Center) is soliciting proposals to provide all supplies, labor, supervision, tools, and equipment necessary to fabricate, repair, and supply dental products on an "as needed" basis for the Tomah VA Medical Center, Tomah, WI. Contract Period of Performance is anticipated for the period July 1, 2007 through September 30, 2007, with four (4) one year option renewal periods. Option periods are exercised at the sole discretion of the Government. Contract type is firm-fixed unit price. All qualified offerors/firms who believe they can satisfy this requirement are invited and encouraged to provide an offer. Offers, including options, shall be evaluated for fair and reasonable pricing, technical qualifications/capability to perform services, past performance, and status as a Service Disabled Veteran Owned Small Business. The Government anticipates award of a commercial item contract to the responsible offeror whose offer provides the best value to the Government. The small business size standard for North American Industry Classification System (NAICS) code 339116 is 500 employees. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certifications-Commercial Items and to submit all requested documentation as listed in the instructions to offerors. Electronic version and hard copy of solicitation will be available on or about May 25, 2007. Date for receipt of offers is June 15, 2007 at 3:30 P.M. CDT.
