- The GLAC (Great Lakes Acquisition Center) is soliciting quotations to provide Home Infusion Therapy Services for eligible veteran located within the Veterans Integrated Service Network (VISN) 12. Offeror must be accredited by a National accreditation organization(s) and Medicare certified and licensed by the state/states in which service is provided and remain in compliance throughout the duration of the contract. VA will conduct the procurement according to a combination of FAR Parts 12 and 15 and under Public Law 104-262 and Title 38 USC 8153. This procurement is a competitive single award contract based on NAICS code 621610 with the small business size standard of $12.5 million. Period of performance is October 1, 2007 (or date of award) through September 30, 2008, with four one-year options to renew, subject to the availability of funds. Proposal documents are available for downloading from www.fedbizopps.gov. Electronic offers or fax proposals will NOT be accepted.
