Description

This is a combined synopsis/solicitation for commercial items requirements prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the ONLY solicitation; a written solicitation will NOT be issued. The Department of Veterans Affairs (VA), Southern AZ VA Health Care System (SAVAHCS), Clinical Care Support Service Line, Prosthetics, and Primary Care, Cardiology Lab, 3601 S. 6th Avenue Tucson, Arizona, intends to negotiate multiple Blanket Purchase Agreements (BPA) resulting from this solicitation. Blanket Purchase Agreement shall include the following items: FDA Approved Implantable Cardiac Biventricular Resynchronization Devices, the appropriate leads, and support services from the manufacturer's representative during and post implantation. The contract period shall be for one (1) base year from date of award with the Government having the option to exercise one (1) six month option for a total of one year and six months. The establishment of a BPA with a respondent in no way obligates the government to issue any calls against the BPA. Ordering requests will be made by the COTR on an as-needed basis. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. The applicable NAICS code for this requirement is 339113 size standard 500 employees. The Contractor shall provide a complete copy of the Offeror Representations and Certifications-Commercial Items (FAR 52.212-3 Nov 2006) with its offer or complete the annual representations and certifications electronically at http://orca.bpn.gov. Many of the provisions and clauses cited in this solicitation are incorporated by reference, FAR 52.252-2 (FEB 1998), to the appropriate section of the FAR and VAAR. A review of these provisions and clauses will be necessary for you to understand all aspects of the solicitation.
