- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ) for purchase request FM480071300066. The solicitation document and FAR incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-14 and Defense Federal Acquisition Regulation Supplement, Defense Change Notice (DCN) 20061219. The proposed acquisition is reserved for 100% small business. The NAICS code is 423450 and the small business size standard is 500 employees. The 1st Contracting Squadron has a requirement for a new or refurbished LuxarCare Laser or an equal item for 1 MDG/Medical Logistics, 73 Nealy Ave Bldg 256, Langley AFB, VA 23665. The following contract line items are applicable to this solicitation: CLIN 0001 Item # LC-LX20SP-R, Luxar LX-20SP CO2 Surgical Laser, QTY 1 Package to include: Item # LC-Sign, Laser-in-use sign, QTY 1 Each; Item # LC-Glasses, 10,600nm Laser safety glasses, QTY 3 Each; Item # LC-Masks surgical masks, 1 Box; Item # LC-F-1.5, 1.5 Meter flexible fiber hollow waveguide, QTY 1 Each; Item # LC-F-1.0, 1.0 meter flexible fiber hollow waveguide, QTY 1 Each; Item # LC-H-ST, Straight hand pieces, QTY 2 Each; Item # LXH-A105 Angled hand pieces, QTY 1 Each; Item # LXT-C5, 0.8mm spot size ceramic focusing tips 5mm, QTY 5 Each; Item # LXT-C10, 0.8mm spot size ceramic focusing tips 10mm, QTY 10 Each; Item # LXT-C10SSD, 0.4mm small spot focusing tips 10mm, QTY 5 Each.
