Description
The Hefner VAMC, Salisbury, NC is seeking the services of an Architect-Engineer firm to design a two story addition to the back of the main medical center building (Building 2) totaling 9,000 sq ft of new space.  This will entail the construction of a third and fourth floor onto a previously approved Phase I project that will create a first and second floors. The overall square footage includes the addition of 4,500 square feet of new floor space and approximately 9,977 sq ft of renovated floor space on the third floor , and 4,500 sq ft of new floor space and approximately 5,087 sq ft of renovated floor space on the fourth floor associated with this project.  The project will provide space for General and Specialty Surgery Programs (i.e., Orthopedics, Podiatry and ENT), Endoscopy, Infusion Clinics,  Anesthesia/Pain Management clinics, and much need Surgery enhancements (Integration of Phase I and II Recovery Units, addition of 2 new OR's and Pre-op). This is not a request for proposal. This proposal is unrestricted; both small and large firms within a 150 mile radius of Salisbury (Rowan County) North Carolina may submit. Firms submitting SF 330s shall provide two copies to the attention of Jolette Morrison by 3PM on June 27, 2007. The selected firm shall provide all necessary labor, material, equipment and architectural and engineering services as required to prepare complete contract drawings, specifications, technical reports, and cost estimates through construction of the project. Design shall be in accordance with all applicable requirements of the VA construction standards, space criteria, NFPA, NEC, OSHA and local, state and federal regulations. Reviews at 25%, 75%, 99% and 100% are required. All construction documents are to be provided electronically in the current version of AutoCad and MS Work. NAICS code 541310 is applicable to this procurement. Faxed copies of SF 330 are not acceptable. The SF 330 form may be downloaded at www.gsa.gov/forms. Government-wide Number Note 24 is applicable.
