I.H.; 10 Center Drive; Bethesda,, MD 20892 LINE ITEM 01: 12 Month Preventative Maintenance on 10 ea Follett 50 series ice mach ines. Preventative Maintenance will be done twice per year, per machine. The P.M. will include inspection, check operat ion of machine, cleaning and filter change. LINE ITEM 02: 12 Month Preventative Maintenance on 30 ea. Follett 25 series ice mac hines. Preventative Mainteanace will be done twice per year, per machine. The P.M. will include inspection, check opera tion of machine, cleaning and filter change. LINE ITEM 03: 12 Month Preventative Maintenance on 30 ea. Laboratory grade refriger ators/freezer. 20 ea Jewett PR-37-18, 6 ea Revco UGL232018 and 4 ea Aeigis Scientific RG-22. Preventative Maintenance on the 30 refrig erators/freezers will be done twice per year, per machine. The P.M. will include inspection, check operation of machine , cleaning and filter change. LINE ITEM 04: 1 Cost Service calls as needed for ice machines, refrigerators and freezers in the Clinical Center. Include your regular servic e rate/hour, overtime rate/hour and travel charge per trip, per service call. ADDITIONAL PROVISIONS AND REPRESENTATIONS: Contractor is to provide quote for preventative maintenance on equipment above. (This does not include repairs and repai r parts). Service calls: Contractor must be able to do all repairs, e lectrical and plumbing work associated with the ice makers, refrigerators and freezers. This may also include removal an d installation of equipment. Contractor must list regular ca ll rates/hours, overtime rates/hours and travel rates for al l service calls. The contractor is expected to be the point of contact for all repairs of the listed equipment and other refrigerator/freezer repairs in the Clinical Center. This w ill be done as is needed. Contractor will respond to all ser vice calls within 24 hrs. Monday - Friday between the hrs. o f 7 a.m. - 4:30 p.m. except for federal holidays. Contractor must be an authorized warranty service agent and authorized sales agent for Follett Ice Company. All replace ment parts must be original manufactured parts.
