- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Bay Pines VA Healthcare System, Bay Pines, Florida intends to purchase an upgrade to the Cardiff Teleform Computer Software for Quality Systems. This acquisition is set-aside 100% for small business concerns. The associated North American Industrial Classification System (NAICS) code for this procurement is 511210 with a business size standard of $23 Million. The Bay Pines VA Healthcare System requires the following items listed below. Please provide a unit price, total amount, and total price for the following items: 1 Pre V9 Enterprise to V10 Workgroup QTY 1 UNIT EA UNIT PRICE________ AMOUNT ________ 2 Upgrade Reader V8 to current V10 Reader QTY 1 UNIT EA UNIT PRICE________ AMOUNT ________ 3 Upgrade Designer V8 to current V10 Designer QTY 1 UNIT EA UNIT PRICE________ AMOUNT ________ 4 Upgrade Verifier V8 to current V10 Verifier QTY 4 UNIT EA UNIT PRICE________ AMOUNT ________ 5 Teleform Maintenance Agreement-Annual tech support North/South American Support QTY 1 UNIT YR UNIT PRICE________AMOUNT ________ TOTAL PRICE ____________ Potential sources are invited to submit quotes, provided they are accompanied by product literature. Offerors shall include the maintenance schedule hour of operations. The delivery date will be 30 days after award. The RFQ number VA-248-07-RQ-0077 shall be clearly identified on all submitted materials. The following provisions apply to this acquisition: 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items. The specific evaluation criteria included in paragraph (a) are: 1) price 2) technical capability 3) past performance - Past performance and technical capability when combined are equal in importance to price. Please submit 2 references with quote. All Offerors shall include a completed copy of 52.212-3 Offeror Representations and Certifications - Commercial Items. All offerors shall also ensure that their company is properly registered with the Central Contractor Registration (CCR) at http://www.ccr.gov. The following clauses also apply to this acquisition: 52.212-4 Contract Terms and Conditions - Commercial Items; FAR 52.204-7 Central Contractor Registration; FAR 52.247-34 FOB Destination.
