- Prior Solicitation No. 600-872-07LR for the same subject has been cancelled. This Request for Information (RFI) replaces the prior solicitation. The new RFI Number is: VA-262-07-RI-0060LR. Additionally, the announcement type is now Sources Sought Market Survey. Those that previously responded to the old solicitation number will need to provide a response to the new RFI, referencing the new number. Sources Sought Market Survey Notice is to help determine the interests, identify sources, and firms with the capabilities with regard to an upcoming acquisition for a new Capitated CBOC. This notice does not constitutes nor restrict the Government to an ultimate acquisition approach; however, the information gathered will contribute to the decision as to how we approach this acquisition. This Market Survey is being conducted so that we may determine the number of competitive firms that can provide these services. The Department of Veterans Affairs, Long Beach Healthcare System in Long Beach, CA, has a requirement for Capitated Community Based Outpatient Clinic (CBOC) services in the South Orange County area for a newly established CBOC. The contractor is to provide primary care and mental health services on a full service - all inclusive capitated rate method of payment. The annual capitated rate shall represent a Firm-Fixed Price, Indefinite Delivery, and Indefinite Quantity (IDIQ) all-inclusive reimbursement rate for the performance of all contract services. The contractor facility must provide services exclusively to veterans only. The contract will include a base year (one year from date of award), with four (4) one-year renewal options that can be exercised at the option of the Government. The total duration of the contract will not exceed 5 years. The solicitation will be issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR Part 15. NAICS is 621498, Size Standard $9.0 Million. At this time, there is no solicitation to be issued and will be forthcoming. Interested firms having the skills and capabilities necessary to perform the stated requirements are invited to provide information to contribute to this market survey/sources sought. Responses from potential sources must include the following information and are limited to 10 pages or less: 1. General Information of the company name, address, phone number, fax number, e-mail address(s), & company web site. 2. How long has your company existed. 3. What services does your company primarily provide. 4. Does your company have experience with Dept. of Veterans Affairs capitated CBOC program? 5. What relevant Government and commercial experiences does your company have related to establishment of CBOCs? 6. Describe how you intend to provide the Capitated CBOC services for our Veterans. 7. Describe your company ownership composition listing the name of individuals that control and have authority to enter into a contract with the Government and their percentage of ownership of your company. 8. Will your company be joint venturing with another company? If so, list the name and provide the company's general information, their responsibilities, and authority of managing of the contract. 9. Is your company accredited by Joint Commission Accreditation of Hospital Organizations (JCAHO)?
