- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as an invitation request for proposal and incorporates provisions and clauses in effect through Federal Acquisition Circular. This notice is set aside to small business. NAICS code 339950. Requirement is for one (1) each at the following locations: A). J-Wing of Hospital (20 foot diameter-Outside): remove existing logo or cover old logo. Install new LOGO complete with design ART. Color shall be the same as shown in the design ART provided. The new LOGO shall read: US NAVAL HOSPITAL GUAM and SERVICE WITH HONOR as per Design Art provided. Complete installation of new LOGO shall be contractor?s responsibility. B). J-Wing inside Quarterdeck Lobby (70 1/4 inches diameter - Inside): remove existing logo or cover old logo. Install new LOGO complete with design ART. Color shall be the same as shown in the design ART provided. The new LOGO shall read: US NAVAL HOSPITAL GUAM and SERVICE WITH HONOR as per Design Art provided. Complete installation of new LOGO shall be contractor?s responsibility. C). J-Wing in XO boardroom behind CO seat (31 1/4 inches diameter - Inside): Install new LOGO complete with design ART. Color shall be the same as shown in the design ART provided. The new LOGO shall read: US NAVAL HOSPITAL GUAM and SERVICE WITH HONOR as per Design Art provided. Complete installation of new design ART LOGO shall be contractor?s responsibility. D). J-Wing in Command Conference Room (31 1/4 inches diameter - Inside): Install new LOGO complete with design ART. Color shall be the same as shown in the design ART provided. The new LOGO shall read: US NAVAL HOSPITAL GUAM and SERVICE WITH HONOR as per Design Art provided. Complete installation of new design ART LOGO shall be contractor's responsibility. E). Branch Medical Clinic Bldg. 6 (11 foot diameter - Front of building) : Remove existing logo or cover old logo. Install new LOGO complete with design ART. Color shall be the same as shown in the design ART provided. The new LOGO shall read: US NAVAL HOSPITAL GUAM and SERVICE WITH HONOR as per Design Art provided. Complete installation of new LOGO shall be contractor's responsibility. F). Branch Medical Clinic Bldg 6 (4 foot diameter - East End Wall): Remove existing sign LOGO. Clean and paint wall to receive new design LOGO. Custom fabricate new design LOGO on mounting studs and 1/8 inch aluminum template 3/4" thick. Install new LOGO complete with design ART. Color shall be the same as shown in the design ART provided. The new LOGO shall read: US NAVAL HOSPITAL GUAM and SERVICE WITH HONOR as per Design Art provided. Custom fabricate individual cut letters (Metal or PVC) to read, "BRANCH MEDICAL CLINIC" and install logo. Complete installation of new LOGO and lettering shall be contractor's responsibility. G). Branch Dental Clinic (4 foot x 89.5 inches concrete sign is to be completely rehabilitated): Remove all lettering and symbols from sign. Prepare, prime and paint according to MRACS standards. Paint scheme shall be approved by NH Guam, Facility Manager. Custom fabricate new design LOGO on mounting studs and 1/8 inch aluminum template 3/4" thick. Install new design logo in the middle of the existing concrete signage. Install new LOGO complete with design ART. Color shall be the same as shown in the design ART provided. The new LOGO shall read: US NAVAL HOSPITAL GUAM and SERVICE WITH HONOR as per Design Art provided. Provide and install the Dental corps symbol (oak leaf with two acorns) and the Medical Cadusa. Provide samples to NH Guam, Facility Manager before installation. Custom fabricate individual cut letters (Metal or PVC) to read, "BRANCH DENTAL CLINIC" and install below logo. Font approval will be made by NH Guam, Facility Manager. Complete installation of new LOGO, symbols and lettering shall be contractor's responsibility. Contractor shall provide the following: a complete design logo based on the U.S. Naval Hospital design Art provided, all labor and materials to complete the LOGO design signs, responsible for removing existing sign and repairing/patch holes prior to installation of new design logo, responsible for refurbishing wall prior to installation of new design logo, install the new sign logo. The U.S. Naval Hospital shall be responsible for authorization and base pass access of the contractors during the project. The following clauses apply to this acquisition 52.212-1, Instructions to Offerors-Commercial, 52.212-2, Evaluation-Commercial Items (evaluation based on price and delivery, past performance), 52.212.3, Offeror Representations and Certifications-Commercial Items ( please include a completed copy with your offer)., 52.212-4, Contract Terms and Conditions-Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items.
