- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quotation is being requested and a written solicitation will not be issued. Solicitation number F2FT027115A100 is hereby issued as a Request for Quotation. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This is a 100% set-aside for small business. The North American Industry Classification System (NAICS) code for this project is 339111 with a size standard of 500 employees; the SIC code is 3821. The contractor shall provide all labor, equipment, materials, tools, parts, shipping, and all other items necessary to provide the following items: Line Item 0001: MFR: ParvoMedics Inc. Part# TRUEONE-2400 = Metabolic Measurement System with 2-way non-rebreathing T-shaped valve with saliva trap, polar heart rate interface, and 2 face masks OR EQUAL (Quantity 1 Set ). FOB: Destination for delivery to 23 AST BLDG 99547 665 RED HORSE RD. HURLBURT FIELD FL 32544. Please include the lump sum amount including all shipping and handling charges for FOB Destination. The following provisions and clauses apply to this acquisition and will remain in full force in any resultant award: FAR 52.212-1, Instructions to Offerors?Commercial Items. It is amended to read: Offeror shall submit signed and dated offer to 16 CONS/LGCB, Attn: A1C Timothy Thulin, 350 Tully St, Hurlburt Field FL 32544-5810 by Thursday, 24 May 2007 at or before 12:00pm local time.
