- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is being issued as a Request for Proposals. The provisions and clauses are those in effect though Federal Acquisition Circular FAC 2005-16 dated 22 Mar 07. All firms or individuals responding must be registered with Central Contractor Registration (CCR). This acquisition is solicited 100% small business set aside. The associated NAICS code is: 621999. Estimated period of performance is 1 Jun 2007 to 30 Sept 2008. Womack Army Medical Center Contracting, Ft Bragg, North Carolina intends to award a firm fixed price contract for Emergency Department Documentation Templates. To be considered for award, offerors shall submit a quote for paper record templates Physician and Nurse (Period of Performance - 1 Jun 2007 thru 30 Sept 2007); and Physician, Nurse and Discharge paper record templates (Period of Performance - 1 Oct 2007 thru 30 Sept 2008) and must have the ability to contribute to business flow by allow ing entry of patient demographic information, customizable forms and interface, utilize minimum amount of computer space, discharge instruction capability and support timely accurate medical record coding. Proposals will be evaluated based on price and tec hnical capabilities, where price will be considered significantly more important.
