Description
STAT LAB SCOPE OF WORK The Contracting Laboratory shall perform, or arrange to have performed, a wide range of routine, stat and special tests. All laboratory services will be performed at the Contracting Laboratory's facility or a reference laboratory specifically requested or as meets the following requirements. The Contracting Laboratory shall notify the Manchester VA Medical Hospital of any change in reference laboratories. The Contracting Laboratory agrees that all reference laboratory services to be provided meet the requirements of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO), the Clinical Laboratory Improvement Act of 1988 (CLIA), CAP or any other applicable accrediting agencies. The Contracting Laboratory shall submit a monthly report of usage and charges associated with referred tests (and CPT codes) to the Manchester Laboratory Manager. Billing data provided from the Contracting Laboratory shall be in computerized form in sufficient detail for the Manchester VA Medical Hospital to bill Medicare, Medicaid or other payers. Identified problems during the contract term shall be resolved mutually between the Contracting Laboratory and the Manchester VA Hospital. Whenever necessary, the Contracting Hospital Director of Laboratory Services and the Manchester Medical Center Laboratory Manager shall meet to discuss the quality and appropriateness of services and to resolve identified problems. Quality control and Quality Assurance reports should be furnished upon request. The Contracting Laboratory shall provide a complete test catalog, available in hard copy, which shall include specimen requirements, turn-around times, STAT availability, critical values, CPT CODE and test pricing. The Manchester VA Hospital shall be responsible for the timely and proper gathering of all specimens and the transportation for delivery to the Contracting Laboratory. The Contracting Laboratory will perform routine testing as needed 24 hours per day/ 7 days per week. Specimens for STAT testing will be processed with no additional charge to the Manchester VA Medical Center. In the event that the Contracting Laboratory is able to determine through visual inspection prior to testing a specimen that its handling has not compiled with protocol and valid test results are not possible, the Contracting Laboratory shall not conduct that test and shall immediately notify the Manchester VA Medical Center for notification of rejection. The Contracting Laboratory must provide information on the availability of on-line lab result and identify system requirements and costs. The Contracting Laboratory is responsible to provide hard copy results. All laboratory reports must include the Laboratory's name that performs the tests, laboratory specific Accession number, the date and time a specimen was collected, received by the laboratory and completed, technologist's initials, and the pathologist's review, where appropriate. The Contracting Laboratory must provide running history for lab results by patient, when requested. Appropriate requisitions forms and unique supplies needed for specialized collection of samples are to be supplied by the Contracting Laboratory at no additional charge. Routine tests are to be performed on the day of receipt, seven (7) days per week. All STAT tests will be performed within (1) hour of receipt of specimen. The Contracting Laboratory shall provide emergency services for evenings, nights, weekends and holidays, and the Contracting Laboratory shall phone critical results and stat results and then print those results to a Manchester VA Urgent Care printer. BASED ON THE ABOVE TURN AROUND TIME-THE CONTRACTOR MUST BE LOCATED WITHIN A 5 MILE RADIUS OF THIS MEDICAL CENTER. STAT and Critical value test results or those requested to be called, will be performed and reported by established turn around times and critical value policy to the Manchester VA Medical Center, phoned immediately followed by a fax and printed copy. The Contracting Laboratory shall agree to notify the NH Division of Public Health when the findings indicate a disease reportable to the State of New Hampshire. The Contracting Laboratory shall provide the anti-microbial susceptibility summaries when requested and or on an annual basis. The Contracting Laboratory shall certify and ensure that all employees, Sub -Contracting Laboratory, officers or agents do not use Protected Health Information received from the Manchester VA Medical Center that would constitute a violation of any applicable provision in standards set forth in the Health Insurance Portability and Accountability ACT (HIPAA).
