- Description
- Modification to Response Date. The National Hansen's Disease Programs (NHDP) has a requirement for pedorthist services. The services include (1) construction, modification and repair of custom molded sandals; (2) modification and repair of prefabricated sandals and commercially-available shoes; (3) repair of commercially-available and custom shoes; (4) construction of foot models from various molding medium and adjust foot models; (5) construction and modification of total contact orthoses; (6) adjustment of belts and prosthetic harnesses; (7) assist in fabrication of total contact plaster casts and posterior walking splints; (8) consultation with therapists and health care professionals regarding shoe and orthotic indication and fabrication; (9) educate and train visiting health care providers in sandal construction, shoe modification, construction of foot models and adjustments and total contact orthoses; (10) participate in seminars by demonstrating sandal and orthoses construction; (11) maintain records of work orders, shoe purchase orders, patient files, and monthly reports in paper and electronic formats; (12) track and order materials and supplies and (13) provide assistance to Occupational and Physical Therapy departments in maintaining and fabricating stock of frequently used clinical items. Services are required 20-24 hours per week from date of award through September 30, 2007 with 2 one-year options. Services shall be provided at the NHDP site in Baton Rouge LA. This is a 100% small business set-aside, restricted to small businesses eligible to receive award. The RFP will be posted at the Government website at www.fbo.gov under HHS, Health Resources and Services Administration, BPHC/National Hansen's Disease Programs, Posted Dates on or about March 28, 2007. It is the offeror's responsibility to monitor the site for amendments to the solicitation. The NAICS code for this requirement is 339113. Solicitation will be conducted under Federal Acquisition Regulation (FAR) Part 12 and 13. The closing date for receipt of proposals will be approximately thirty (30) days after issuance/posting of the RFP.
