- Eisenhower Army Medical Center, Fort Gordon, GA, has a requirement to establish an Orthopedic Musculoskeletal System Blanket Purchase Request (BPA) inclusive of the following: elbow plates, distal radius plates, humeral rods, headless screws and tota l shoulder plates. The Government intends to issue a Sole Source BPA to the Suggested Source: Southern Orthopaedics, 1100 Hammond Drive NE 410A, Atlanta, GA 30328-8198. This requirement is unique to patient care and will be established for procurement of Orthopedic Musculoskeletal Systems on an as need basis. The contents of this Blanket Purchase Agreement are established in support of the Military Healthcare Community. Orthopedic Musculoskeletal Systems are readily available in the commercial industry; however, it is essential that a physician utilize orthopedic musculoskeletal systems for which he/she is highly trained to maximize medical risk to patients. Services ordered under the established BPA will be for a period of 5 Years or until the Governmen t determines that a bona fide need no longer exist. This is not a request for quotation. However, vendors with similar or like Orthopedic Musculoskeletal Systems must submit technical capabilities inclusive of past performance information, delivery schedu les and current price listing to be evaluated by the Governments Technical Representative for consideration of future requirements.
