- Contractor will provide the services of one Licensed Physical Therapist Assistant (PTA) to the Tomah VA Medical Center, 500 East Veterans Street, Tomah, WI for the estimated period April 9, 2007 through June 15, 2007, with option to extend through September 30, 2007. The normal work schedule will be Monday through Friday from 8:00am to 4:30pm., with a ½ hour non-paid lunch break. Federal holidays are excluded from normal work schedule hours of operation. Contract provider will provide (8) hours per day (40) hours per week of both inpatient and outpatient care services. This procurement is unrestricted and is open to all offerors. North American Industry Classification System (NAICS) Code is 621399, size standard $6.5 million. Offerors are reminded to complete all sections of the solicitation, including the Offeror Representations and Certification-Commercial Items and to submit required all required information.
