- This is an amendment to the solicitation for delivery service for government-owned durable medical equipment for beneficiaries within VISN 10. Amendment #5 is issued to clarify CLINs 0001; 0002; 0003; and the omission of storage space. The geographic area is comprised of, but not limited to, the following: Louis Stokes Cleveland VAMC, Wade Park Unit, 10701 East Blvd., Cleveland, OH 44106 and Brecksville Unit at 10000 Brecksville Rd, Brecksville, OH 44141, VAMC Cincinnati 3200 Vine St., Cincinnati, Ohio 45200: Chalmers P. Wylie Veterans Clinic, 542 Taylor Rd., Columbus, Ohio 43203; Dayton VAMC, 4100 West Third Street, Dayton, Ohio 45428 and Chillicothe VAMC, 17273 State Route 104, Chillicothe, Ohio 15601. This is a 100% Small Business (SB) set-aside. The North American Industrial Classification Standard (NAICS) is 621610, applies to this procurement with the business size standard of $12.5 million. The proposed contract action is for services for which the Government intends to solicit and negotiate.
