- Description
- The VISN 9 Acquisition Service Center intends to award a contract under the Hub Zone Act of 1997 (15 U.S.C. 657a) Sole Source Award authority to LRB&T Corporation, Lenore, WV, for Community Based Primary Care Services to be provided in Logan, WV. The period of performance will be 6-1-07 through 5-31-08 with one renewal option period of one year. Established Primary Care Service Providers must submit in writing sufficient evidence demonstrating their ability provide Primary Care Services to a minimum of 400 Veterans in the Logan WV area on or before April 11, 2007, 2:00pm CT. Contractor's facility must be located within the city limits of Logan WV or no more than 5 miles outside the corporate Logan WV city limits. Contractor MUST be Hub Zone Certified by the Small Business Administration. Three complete sets of the contractor's information including the Hub Zone Certificate must be sent to the Acquisition Service Center, VISN 9, 3400 Lebanon Pike, Murfreesboro, Tennessee 37129. Upon receipt of this information the contracting officer will determine whether to conduct a competitive Hub Zone procurement or proceed with sole source negotiations.
