Description
The CDC is contemplating awarding a firm-fixed price contract utilizing the Simplified Acquisition Procedures for the purpose of convening a forum with and among faith leaders, seminarians, and seminary students about how faith leaders can raise awareness about the impact of HIV/AIDS on African Americans. The title of the project is HIV/AIDS Faith-based Project. Over the last year, the Centers for Disease Control and Prevention (CDC) has conducted a host of activities as a part of our heightened response to the HIV/AIDS epidemic in African American communities. The Division of HIV /AIDS (DHAP) seeks to prevent the spread of HIV/AIDS by tapping the expertise of various sectors, including the faith community and its leadership, to mobilize broader community action; increase opportunities for diagnosing and testing HIV; improve program services; and prevent HIV infections through effective interventions. The CDC has long recognized the essential role that the faith community plays in influencing knowledge, attitudes, beliefs, and behaviors relating to health. As a result of a faith-based consultation completed by DHAP in 2006, CDC found that faith leaders expressed a strong desire for education to help them address HIV/AIDS effectively so that they may play an important role in efforts to address the HIV/AIDS epidemic in the African American community. The project will enable faith leaders from African American faith denominations to acquire and develop the skills to better raise awareness about the impact of HIV/AIDS on African Americans, and to develop ways to inform and inspire their peers and congregants to increase HIV prevention. The period of performance is the April 11, 2007 to August 30, 2007. The minimum qualifications include: ? Must have access to church denominations that reach and serve large numbers of African Americans ? Must have access to seminary faculty, staff, and students who are familiar with; have expert knowledge and experience with those church demonitions that serve large numbers of African Americans ? Must have over 10 years of expereince working with, training, and mentoring church denominations that reach and serve large numbers of African Americans ? Must have onsite facilites such as auditorium, classrooms, meeting rooms located in a geographic region of the US that is both readily accessible by participants and is located in proximity to a large number of African American church denominations within it's district and in adjoinging districts and states This is a combined synopsis/solicitation. The date specified for receipt of offer, the proposal is March 28, 2007 12:00 p.m. EST. STATEMENT OF WORK Title of Project: HIV/AIDS Faith-based Forum Project I. Background and Need ? It has been more than 25 years since the first U.S. diagnosed case of AIDS. Although efforts have been made to prevent the spread of HIV/AIDS in the U.S., Americans continue to be affected by increasing HIV/AIDS prevalence rates. The African American community ? more than any other racial or ethnic group - has been severely impacted by the HIV/AIDS epidemic. Even though African Americans comprise only 13 percent of the U.S. population, they accounted for half of the new HIV/AIDS cases diagnosed (in 33 states) and more than half of the people who have died from AIDS. The HIV/AIDS Faith-based Forum Project is a vehicle through which, faith leaders from African American faith denominations shall acquire and develop the skills to better raise awareness about the impact of HIV/AIDS on African Americans, and to develop ways to inform and inspire their peers and congregants to increase HIV prevention. II. Project Objective ? The objectives of the HIV/AIDS Faith-based Forum Project are to: ? Disseminate prevention messages regarding benefits of early HIV testing, prevention, and care; ? Address the stigma associated with HIV/AIDS by promoting open dialogue about the disease, and support for those persons living with HIV and AIDS; and ? Increase the ability of faith leaders to promote HIV prevention awareness within their congregations and communities. The HIV/AIDS faith-based forum(s) shall allow participants to engage in panel discussions about ways to break the silence about HIV/AIDS, challenge stigma, and promote HIV testing and diagnosis in African American churches as a means of reducing HIV/AIDS in African American communities. HIV/AIDS faith-based forum participants shall discuss ways they, as African American faith leaders, can encourage care and compassion for people living with HIV/AIDS. III. Scope of Work ? The contractor shall convene one forum with and among faith leaders, seminarians, and seminary students about how faith leaders can raise awareness about the impact of HIV/AIDS on African Americans. This contract shall provide services associated with convening the HIV/AIDS faith-based forum in four areas: Task 1 ? Project development and management Task 2- Participant identification and engagement Task 3- Project coordination Task 4- Development of communications strategies, materials, and tools Task 1: Project development and management. The Contractor shall use its expertise in conducting faith-based forums for and with African American faith leaders to inform the development and management of the HIV/AIDS Faith-based Forum Project. Task 2: Participant identification and engagement. The Contractor shall use its expertise to identify and engage current and potential African American faith leaders, including those from, or serving high HIV/AIDS impacted communities to participate in the HIV/AIDS faith-based forums. Task 3: Project coordination. The Contractor shall coordinate all aspects of each HIV/AIDS faith-based forum to be conducted on April 30, 2007 as a follow up to the launch of CDCs Heightened Response to the Ongoing Crisis of HIV/AIDS among African Americans that was conducted March 8, 2007. Task 4: Development of communications strategies, materials, and tools. The Contractor shall coordinate all aspects of communication with forum participants, members of the African American faith community, and other stakeholders as identified. The Contractor shall use its expertise to develop forum materials and tools for use by up to10 panelists and/or guest speakers and up to twenty-five (25) attendees participating in the HIV/AIDS faith-based forum. Materials and tools shall include objective driven curriculum, agenda, books, articles, templates, reference material, and other items that can facilitate learning and skills building. The materials will be produced as templates, and will be maintained for print and copy for any and all future faith-based forums. IV. Technical Requirements ? Task 1.0: Project development and management. Subtask 1.1/Project Plan: The Contractor shall develop and submit for approval a project plan proposal to detail the approach for developing the HIV/AIDS faith-based forum and managing all tasks within this contract?s statement of work (SOW). The major topics which the plan shall include are: 1) identification and engagement of current and potential African American faith leaders, including those from or serving high HIV/AIDS impacted communities to participate in the HIV/AIDS faith-based forum; 2) coordination of the HIV/AIDS faith-based forum project; 3) developing and managing training materials and tools; and, developing communications strategies and tools. The contract shall include in the plan a suitable approach to evaluate the plan and ensure quality assurance and performance. Deliverable: The Project Plan shall be delivered not later than two weeks prior to convening the forum. Subtask 1.2: Project Management: The contractor shall participate in appropriate implementation management activities to include, but not limited to: 1) conducting an analysis of the CDC Consultation on Faith and HIV Prevention located at http://www.cdc.gov/hiv/resources/other/PDF/faith.pdf, and use that information to inform and structure the HIV/AIDS faith-based forum , as well as formulate target outcome measures for the HIV/AIDS faith-based forum project; 2) using appropriate methods and formats to evaluate, monitor quality, record, document, and disseminate project activities and outcomes; 3) meeting with DHAP Prevention Partnerships Office management and other staff, as appropriate, to clarify methodology and delivery schedule, delineate roles and responsibilities and establish a communication protocol; 4) participating with the CDC and other staff and/or contractors as appropriate in weekly meetings at a predetermined location; 5) participating in appropriate implementation activities; and, 6) as specified and requested by the Project Officer, the Contractor shall collaborate and coordinate activities with other PPO contractors and with CDC staff in various other centers/institute/offices (CIOs) who provide support services. Deliverable: The contactor shall provide meeting summaries containing minutes and/or documentation of each CDC requested meeting (telephone or face-to-face) transcribed as text within an MS Word document. All documentation shall be in narrative format including, at a minimum, date, time, and location, names of participants, discussion, recommendations, and next steps. Summaries shall be delivered not later that one week after completion of the meeting. Task 2.0: Participant identification and engagement. Subtask 2.1/Forum participant identification and engagement Develop and implement a strategy to identify African American faith leaders, including those from or serving high HIV/AIDS impacted communities, to attend and participate in the HIV/AIDS faith-based forum. Included among those participants are up to ten (10) panelists and/or designated speakers who shall be tasked to address forum participants. The contractor will work with CDC to identify and implement a selection criteria that will consider panelist/designated speaker level of expertise in the area of HIV/AIDS, experience in developing and communicating strategies. Deliverable: The Forum participant identification and engagement strategy shall be a section within the Project Plan and shall be delivered not later than 2 weeks after award.. The Contractor shalll develop a list of prospective participants for the forum and submit to PPO for approval not later than 2 weeks after award. The Contractor shall develop a list of up to ten prospective panelists and/or designated speakers for the forum and submit to PPO for approva1 not later than 2 weeks after award. Task 3.0: Project coordination. Subtask 3.1/Coordinate all aspects of HIV/AIDS faith-based forum. Apply expertise and knowledge of the target audience to coordinate, in collaboration with CDC, all aspects of developing and deploying the faith-based forums. Project coordination will entail: ? Upon PPO approval, invite prospective attendees, panelists, and designated speakers to attend one forum in Atlanta. ? draft, and edit brochures, letters and other correspondence needed for the forums. ? promote the forums among historically black seminaries, local schools of theology and African American church pastors ? provide printing and production services including videography, photography, duplication services, design and print services for all materials related to the forums ? coordinate and arrange for travel, accommodations, and logistics for up to10 panelists and/or guest speakers and up to twenty-five (25) attendees participating in the forum. Note: No government employees may be included for sponsored travel ? Provide the technical equipment including digital projectors, screens, PA systems and related items needed for each forums. ? Compile and submit presentations, papers, and other written documents arising from the HIV/AIDS faith-based forums to peer reviewed journals for publication, including those pertaining to theology. Deliverable: The Contractor shall provide samples, products, and/or reports as appropriate for each of activities outlined in Subtask 3.1.and as requested and approved by PPO no later than three days after request. Task 4.0: Development of communications strategies, materials, and tools. Subtask 4.1/ Develop communication strategies, materials and tools. Apply expertise and knowledge of the target audience to develop, in collaboration with CDC, communications strategies, materials, and tools that can be used to increase the knowledge, skills, and abilities of faith-based leaders and forum participants serving African American communities. The Contractor shall provide printing and production services for all materials and tools such as videography, photography, duplication services, design and print services used for the forum. Materials and tools shall be gathered based upon identified need, preferred practices or effective strategies for HIV education and prevention in general and within the faith community in particular. Materials should be developed according to target outcomes and principles of adult learning. Materials and tools shall include: ? objective driven curricula, ? agenda, ? books, ? articles, ? templates, ? reference material , and ? other items that can facilitate learning and skills building. Deliverable: The Contractor shall provide materials as requested and approved by PPO and in accordance to corresponding activities delivered no later than three days after request. Subtask 4.2/ Develop materials and tools that can be used to increase awareness of the HIV/AIDS faith-based forum among the faith leaders who serve African American communities. The Contractor shall provide printing and production services for all materials and tools such as videography, photography, duplication services, design and print services used for the forum. Materials to be developed and disseminated may include: ? brochures, ? letters, ? publications, and ? other correspondence that can promote the HIV/AIDS faith-based forum. Deliverable: The Contractor shall provide materials as requested and approved by PPO not later than 3 days after request. V. Reporting Schedule ? A. Activity Reports The Contractor shall provide activity reports at the request of the Project Officer. Activity reports shall include items such as summaries of project planning, management, and coordination, as well as any and all activities related to the HIV/AIDS Faith-based forum. Specific form, format, and content of the reports shall be agreed on by the Contractor and the Project Officer at time of the request. Reports will be compiled as determined by CDC in an appropriate format as agreed upon at time of request. These reports shall be received NLT 7 calendar days after request. B. Final Report The Contractor shall provide the Project Officer and PPO a final report summarizing all HIV/AIDS Faith-based forum activities that will be used to inform any or all future faith-based forums planned and convened by the CDC. The report shall summarize: ? progress toward meeting objectives, milestones, and schedules during the project period. ? problems/opportunities and recommended responses and solutions. The final report shall be delivered NLT the 30 calendar days following the Forum. Specific form, format, content, and delivery schedules of the reports shall be agreed on by the Contractor and the Project Officer. All reports shall be submitted in hard copy and/or electronic formats and on media appropriate for CDC?s use, including word-processing, spreadsheets, and graphic formats. VI. Technical Appoach Please provide not more than three pages of proposed approach to completing the proposed work. Please address understanding of requirements, target audience, subject matter and specific considerations/techniques proposed. III. Personnel Please provide a curriculum vitae/resume for each individual expected to provide 20% or more of the labor under order. Please address education, work experience and any relevant training of the individual. Please explain the role each individual is to play in completing the requirements in the Statement of Work. IV. Past Performance Please provide a list of references who can validate the quality of performance on similar work. Include references for work listed in item I. above, in addition to any others. Please provide the following for each reference, not to exceed a total of five: Name; Employer; Address; Telephone Number/E-mail address; Description of work for which they are provided as a reference and individual?s role.
