- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The purchase request number for this request for quotation (RFQ) is F3G4BA7036A100. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-15. (iv) This acquisition is a 100% small business set-aside. The applicable NAICS code is 611710 with a size standard of $6.5 million. (v) Line Item 0001: Emergency Medicine Risk Reduction Analysis and Training. Description: The program provides a system solution to patient safety and medical errors in the practice of emergency medicine through on-line Continuing Medical Education (CME) for nurses and physicians. The program is also used for the collection, analysis, and reporting of data from Mike O?Callaghan Federal Hospital emergency patients chart audits. (vi) Line Item 1001: Option Year 1 (1 Oct 07 ? 30 Sep 08); Line Item 2001: Option Year 2 (1 Oct 08 ? 30 Sep 09); Line Item 3001: Option Year 3 (1 Oct 09 ? 30 Sep 2010); Line Item 4001: Option Year 4 (1 Oct 2010 ? 30 Sep 2011) (vii) The contract period of performance for the first year will be 1 April 07 ? 30 Sep 07. (viii) The FAR provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) The FAR provision at 52.212-2, Evaluation -- Commercial Items, is applicable to this acquisation. The evaluation factors are technical acceptability and price. Technical acceptability is more significant than the price. Price will not be considered for any offers that are not technically acceptable.
