- All quotes must be submitted on SF1449 with all blocks completely filled out. Quotations shall include proposed unit price, company name with point of contact, address, phone number, fax number, tax identification number, DUNS number, and size status (i.e. small business, small disadvantaged business, woman owned business, HUBZONE, etc.) Please read and provide all requested information. Your signature indicates your intention to comply with the terms set forth in this solicitation. Delivery terms are FOB Destination to Barstow, California. The resulting purchase order shall be firm fixed price. Offerors must be registered with Central Contractor Registration. This requirement is 100% se-aside for small business. See Note 1.
