Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotes (RFQ). Submit written offers (oral offers will not be accepted), on RFQ F2U3147051A100. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-14 and Defense Acquisition Circular 91-13. This procurement is being issued as 100% small business set-aside. The North American Industry Classification System (NAICS) Code is 238290 and the size standard of $12M applies to this procurement. The requirement is provide and install 170 feet of verticle lifeline cable on the water storage tank ladder at Luke AFB, AZ, IAW the Statement of Work. FOB Destination to Luke Air Force Base, Arizona 85309. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items, applies to this acquisition. The evaluation will be based on low price offer. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. To be eligible to receive an award resulting from this solicitation, contractor must be registered in the DoD Central Contractor Registration (CCR) database, NO EXCEPTIONS. The response due date is 5 March 07. Quotes are required to be received no later than 4:00 PM PST, 5 March 07. STATEMENT OF WORK INSTALL WATER TANK FALL PROTECTION 1. SCOPE: The work covered by this statement of work consists in furnishing all plant, labor, equipment, appliances and materials and performing all operations in connection with the installation 170 feet of lifeline cable on the water storage tank ladder, facility 289, and associated equipment and hardware for 2 personnel. 2. INSTALL NEW: The contractor shall install the 3/8 inch vertical lifeline cable in 2 sections. The lower section will be 120 feet long with the bottom termination just above the safety hatch on the leg of the tower. The top termination will allow the climber to remain attached to the cable until they have climbed over the hand rail. The cable grab should remain with the vertical lifeline. The upper section will be 50 feet long and follow the curve of the tank to the top. The bottom termination shall start at the bottom rung of the ladder and continue to the top of the ladder. 3. MATERIAL, EQUIPMENT AND HARDWARE: All material, equipment and hardware must be OSHA approved for fall protection. Material includes the vertical lifeline cable (3/8 inch) and 2 cable grabs with all mounting hardware. Equipment includes 2 OSHA approved fall protection harnesses. 4. TESTS AND TRAINING: After the installation is complete and at such time as the Contracting Officer may direct, the contractor shall conduct an operation test for approval to ensure the equipment is operational and provide training to base personnel. The equipment shall be demonstrated to operate in accordance OSHA standards. The test shall be performed in the presence of the base electricians and utilities technicians. The contractor shall provide all equipment and personnel required for the test and training. 5. WARRANTY: The contractor shall provide a 1-year warranty on all labor and material. PERFORMANCE PERIOD Work under this contract shall be completed within 30 calendar days after receipt by the contractor of written Notice to Proceed.
