- Description
- The Bay Pines VA Healthcare System intends to solicit a service contract for the period April 1, 2007 through March 31, 2008, with the government having the option to renew annually for two (2) years, for the following dental lab services. The Contractor shall provide the necessary personnel and materials to complete all phases of dental prosthetic procedures to include, but not limited to: processing complete and partial dentures, pouring/pinning models, making custom trays, wax rims, set-ups, casting framework, crowns and bridges, repairs, valplast partials, diagnostic wax-ups, custom welding, orthotic devices, orthodontic retainers, special appliances, and custom gold and ceramic work requested in accordance with the written dental laboratory prescriptions from the Dental Service Providers at the Bay Pines VA Healthcare System, 10000 Bay Pines Blvd., Bay Pines, Florida 33744. The contractor will be located in the state of Florida and in the counties of either Pinellas, Hillsborough or Manatee, and shall provide transportation for pick-up and delivery of all dental cases from the Dental Lab on a daily basis, and complete all phases of dental prosthetic procedures in a timely manner. Hard copy solicitations will not be made available. Contractors may obtain a copy of the solicitation and any attachments at the following URL: www.fedbizopps.gov. Contractors are advised that they are responsible for obtaining any amendments. All quotes must be returned to the address in block 9 of the SF 1449. North American Industry Classification System (NAICS) code 621511 applies to this procurement. This is a 100% total small business set-aside. This solicitation is being issued as a Commercial Item Acquisition in accordance with FAR Part 12, and will be issued on or about February 28, 2007 with quotes due on or about March 14, 2007. The anticipated award date is on or before March 30, 2007.
