Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 21-07-237PBJ003 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 05-08. The associated North American Industrial Classification System (NAICS) code for this procurement is 611699 with a small business size standard of $6,500,000. This requirement is a Small Business set-aside and only qualified offerors may submit a quote. The deadline for submitting quotes is: Friday, December 22, 2006 at 1:00 PM EST. The U.S. Coast Guard requires a contractor to provide classroom instruction and field exercise training to Coast Guard personnel in shore confined space entry for a base year plus four optional periods as follows: Base Year: Date of contract Award through 9/30/2007. Location of Training: TRACEN Yorktown, Va. Number of Classes: Three (3) Start Dates (fixed, non-negotiable): 4/27/07, 5/22/07, 9/25/07 Location of Training: TRACEN Petaluma, Ca Number of Classes: One (1) Start Dates (fixed, non-negotiable): 3/5/07 For Both Locations: Duration: 4 Days Maximum Class Size: 25 (Note: only four (4) classes shall be provided in the base year) Option Year 1: 10/1/2007 to 9/30/2008 Option Year 2: 10/1/2008 to 9/30/2009 Option Year 3: 10/1/2009 to 9/30/2010 Option Year 4: 10/1/2010 to 9/30/2011 In each of the four option years, the contractor shall provide five (5) classes each year (3 in Virginia, and 2 in California). Classes shall last 4 days and each class will have a maximum of 25 students. Dates will be provided upon the exercise of each option period, but for planning purposes, the Coast Guard anticipates one class will be offered quarterly, with an additional class offered in the third quarter (for a total of 5 classes each year). COURSE PURPOSE: The purpose of this course is to provide the necessary training to a collateral duty unit confined space program manager to conduct duties per COMDTINST 5100.47: Manage and implement unit confined space entry program. Identify and evaluate the hazards of confined spaces. Inform/train employees of the hazards of a confined space. Prevent unauthorized entry into permit spaces. Implement a written permit-required confined space entry program for authorized entry including use of entry permits and checklists. Identify acceptable entry conditions. Energy isolation, safe out and control or elimination of atmospheric hazards. Re-evaluate space to upgrade/downgrade requirements based on changed conditions. Train students in the practical use and application of four-gas detection monitoring equipment. Ensure availability of necessary equipment and services: Personal protective equipment and respiratory protection, Testing and monitoring equipment, Ventilation, communication, lighting equipment, Other equipment as necessary for safe entry. Annual review of program effectiveness. COURSE CONTENT SHALL INCLUDE THE FOLLOWING: Shore Confined Space Entry & Rescue training reviews Coast Guard policy COMDTINST M5100.47 and OSHA 29 CFR 1910.146, regarding Shore Confined Space Entry and Rescue procedures. Program management and implementation shall be covered including requirements for developing a written Confined Space Safety Instruction. The duties of entrants, attendants, entry supervisor, and rescue team members shall be discussed in detail. Characteristics of Confined spaces shall be explained and methods taught to identify and distinguish a Non Permit required confined space from a Permit required confined space. Risk management of confined space entry is integrated into hazard recognition, evaluation, and control, permitting, and monitoring procedures. Though focusing on shore-confined space, aviation-confined space shall also be reviewed. An overview of confined space test equipment shall be performed with each student, practicing calibration and use functions of the equipment. Students shall perform practical exercises using monitoring equipment to test spaces. Training and Qualifications Sources of additional information and/or assistance. INSTRUCTOR (S) SHALL PROVIDE Student manual. Shall include copies of: OSHA 29 CFR 1910.146, COMDTINST 5100.47, Power-point presentations and other pertinent information pertaining to course material. Provide copy of course training outline to COTR for content review prior to initiation of initial course. All curriculum changes to course shall be approved by COTR prior to implementation. Class Completion Certificate Test instruments (detector tube) and sample media Instruction to be combination of o Lecture with Power Point presentations and Overheads o Videos and Case histories o Exercises Hands-on atmospheric test instrument use Mock confined space permitting and entry Tabletops/Scenarios Test or other means to evaluate student understanding Provide COTR written student self- assessment ratings, student course objectives critiques and student roster Provide student evaluation of instruction Provide COTR with status of Combustible Gas monitors requiring repairs and replenishment of calibration gasses. Room condition should be maintained clean and orderly with all support equipment properly stored at completion of training. COAST GUARD WILL PROVIDE Combustible gas monitors used for demonstration and training Provide Calibration Gasses to support Combustible gas monitors Photos examples of various confined spaces located on Coast Guard facilities ADDITIONAL INFORMATION FOR INSTRUCTORS This training is designed for Shore Facility Confined Space Entry and Rescue application. This course is intended for Coast Guard personnel military and civilian who are assigned to shore facilities having confined spaces and who have been assigned duties as attendants, or entry supervisors with responsibilities to test and monitor confined spaces. Personnel attending this course should have completed the online Coast Guard Confined Space Safety Awareness training prior to enrollment in this course and have a basic understanding of industrial safety practices. Note: This course does not satisfy confined space entry training requirements for Coast Guard Marine Safety, Law Enforcement, Vessels or Small boat repair personnel. QUOTES SHALL BE EVALUATED ON A PASS/FAIL BASIS AS FOLLOWS: INSTRUCTOR (S) QUALIFICATIONS (1) At least 5 years instructional experience in teaching confined space entry. (2)At least 2 years instructional experience in teaching confined space or other safety courses to Coast Guard military and civilian personnel. (3)Familiar with Coast Guard organization and shore and aviation confined space operations. (4)Authority in Coast Guard and policies federal regulations regarding confined space entry and risk management. (5)Experienced in all aspects of confined space entry, rescue and emergency services. (6)Use of the process and principles of risk management in instruction approach. (7)Use of additional instructors (i.e. subject area experts), if needed. Quotes are limited to 10 pages total. Offerors shall propose a firm-fixed-price on a per year basis. An adequate price breakdown must be included in your quote which will indicate course price, travel price, and any other pertinent line-item. The Coast Guard intends to award one firm-fixed-price contract with four option years resulting from this competition. The firm that receives a "pass" in all categories under instructor qualifications and has the lowest price will receive the award. The selected Offeror must comply with the following commercial item terms and conditions: FAR 52.212-1, Instructions to Offerors and 52.212-3, Offeror Representations and Certifications - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items.
