- Request ofr Quote (RFQ), W912JD-07-T-0003, Physician Assistants for the Maine Army National Guard, at the Department of Veterans Affairs - TOGAS, Augusta, Maine for 150 Soliders on 3-4 March 2007 is amended as follows: The purpose of the amendment is to clearify questions and answers from this solicitation. Offer due date has not changed: 15 Feb 07 at 10:00am. 1.Question: Are they going to be a Periodic Health Assessments (PHA) or complete Chapter 3 Physicals. Answer: These will be Chapter 3 Physicals - not PHAs. 2. Question: Specifically which supplies (besides stethescope and sphygomanometer) are the contractor responsible to provide; it seems that the Medical Command (and TOGAS VAMC) provide examination rooms, almost all equipment, forms, etc. related to the r equired procedures. Answer: Hammer Reflex 3. Question: Are both the completion of 150 physicals and the submission of paperwork to Medical Command expected to be completed during the period of 3-4 March 2007. Answer: Yes, The contracts are augmenting organic PA/NSs.
