Description
a. Phase II of cardiac rehabilitation, as described by the U.S. Public Health Service, is a comprehensive, long-term program including medical evaluation, prescribed exercise, cardiac risk factor modification, education, and counseling. Phase II refers to outpatient, medically supervised programs that are typically initiated 1-3 weeks after hospital discharge. Cardiac rehabilitation sites that provide any such cardiac rehabilitation services must hold current certification at the time of award and over the life of the contract. This certification must be issued by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR). Any contractor proposing on this solicitation must be certified in order to be considered for award. Additional information on program certification is available at http://www.aacvpr.org/. b. This contract is for the purchase of patient sessions for phase II cardiac rehabilitation services for patients of the VA Nebraska-Western Iowa Health Care System and its catchment area (see p. 4 for list of catchment area counties). Each cardiac rehabilitation patient session includes all technical and professional components of the service. Phase I cardiac rehabilitation services will be provided by the contract institution in which the cardiac intervention or surgery was provided and will not be subject to the terms of this solicitation. c. The contractor shall provide technical and professional components related to cardiac rehabilitation services for VA patients at their location in accordance with the requirements of this contract. Cardiac rehabilitation services will be provided on a cost per session basis over a maximum two (2) month period. Services provided in connection with a cardiac rehabilitation exercise program may be considered reasonable and necessary for up to 18 sessions. Patients generally receive 2 to 3 sessions per week for 6 to 9 weeks. d. The contractor(s) agree(s) to provide licensed staff trained in cardiac rehabilitation and any other support personnel required. To be comprehensive the program must include a medical evaluation, a program to modify cardiac risk factors (e.g., nutritional counseling), prescribed exercise, education, and counseling. e. Cardiac rehabilitation sites who provide any of the cardiac rehabilitation services must demonstrate current certification by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR) < http://www.aacvpr.org/>. f. The contractor(s) will provide all supplies and equipment required in rendering the cardiac rehabilitation services. g. The contractor(s), under the terms of this contract, will accept VA patients only from the VA Nebraska Western Iowa Health Care System. Cardiac rehabilitation services shall not begin until the contractor receives the official authorization for services from the VA. h. The contractor(s) shall have available for immediate use the necessary cardio-pulmonary, emergency, diagnostic, and therapeutic life-saving equipment as required by the American Association for Cardiovascular and Pulmonary Rehabilitation (AACVPR). i. Contractor(s) must have cardiac surgery capability, and/or cardiologist subspecialty capability on site and available on an emergent basis if needed during the time in which cardiac rehabilitation services are being provided to VA patients. j. The contractor(s) shall document each cardiac rehab session conducted by certified personnel. Documentation should clearly demonstrate patient's response to treatment, the patient's success in meeting treatment goals/outcomes, and progression within the plan the care. Documentation will be readily available to support billed sessions. Patients not able to demonstrate success in meeting treatment goals or outcomes should have their plan of care changed to allow success or be discharged from cardiac rehab services prior to completion of the maximum 18 visits. If after evaluation by certified cardiac rehab personnel, the need is identified for additional services (over the 18 visits) the contractor shall provide to VA a request for re-authorization that includes supportive documentation. Additional visits should not be performed until official authorization is received from the VA. Requests for additional visits will be reviewed by the COTR at the VA. k. The VA agrees to provide patient authorization/request including discharge summary, operative report, and "Release of Information" form.
